How Testosterone Cypionate Affects Oestradiol

Testosterone cypionate aromatises to oestradiol in direct proportion to how much testosterone is circulating, identically to enanthate. Oestradiol rises with dose and with peak concentration, which makes injection frequency the main practical lever. Both excess and deficiency cause symptoms, and crushing it has measurable costs.

The Mechanism

Once the cypionate ester is cleaved, the circulating hormone is testosterone, and the aromatase enzyme converts a fraction of it to oestradiol in adipose tissue, liver, brain, bone and testis. There is no ester-specific aromatisation behaviour: the substrate is the same molecule in both cases.

What determines how much oestradiol you produce:

  1. Total testosterone exposure: more substrate means more product, in a broadly linear relationship.
  2. Peak concentration: aromatase is not saturable at physiological concentrations, so a high peak from a large infrequent injection produces a correspondingly high oestradiol peak. This is why injection frequency changes the oestradiol picture without changing the weekly dose.
  3. Adipose tissue mass: aromatase is highly expressed in fat. Two men on the same protocol with different body compositions will not produce the same oestradiol.
  4. Genetic variation in CYP19A1, the aromatase gene, produces meaningful individual differences.

Why oestradiol is worth protecting rather than minimising: oestradiol in men supports bone mineral density, joint comfort, libido, mood and cognition, and it upregulates hepatic LDL receptor expression. Zmuda (1993, PMID 8487666) gave weightlifters testosterone enanthate with and without an aromatase inhibitor and found every lipid endpoint worse when aromatisation was blocked: HDL fell 20% instead of 16%, apoA-I fell 15% instead of 12%, and hepatic lipase activity rose 38% instead of 21%. Suppressing oestradiol is not a free action.

Expected Changes

Replacement doses (100-200 mg/week):

  • Oestradiol commonly rises into the 100 to 200 pmol/L range (roughly 27 to 55 pg/mL), tracking total testosterone.
  • Most men are asymptomatic across a wide range. There is no single correct number.

Supraphysiological doses (300-600+ mg/week):

  • Oestradiol frequently exceeds 250 to 400 pmol/L (roughly 70 to 110 pg/mL).
  • Bhasin (2001, PMID 11701431) demonstrated that testosterone effects follow a linear dose-response up to 600 mg/week; aromatisation follows the substrate.

Injection frequency changes the picture at the same weekly dose:

  • 200 mg every two weeks produces a much higher peak, and a correspondingly higher oestradiol peak, than 100 mg weekly.
  • McFarland (2017, PMID 29264562) documented stable total testosterone across a full week on subcutaneous weekly cypionate, averaging 627 ng/dL with no adverse effects, which is the pharmacokinetic basis for the more frequent dosing approach.

Timing: Oestradiol tracks testosterone closely and moves within days of a dose change.

Assay matters: Immunoassay oestradiol is unreliable in men at the low end. If a result is being used to justify an aromatase inhibitor, an LC-MS/MS (sensitive) oestradiol is the test to insist on.

Monitoring Guidance

Baseline: Oestradiol before starting, ideally by a sensitive assay, so that later results have a reference point.

On protocol:

  • Recheck 6 to 8 weeks after starting or after any dose change.
  • Sample at trough alongside total testosterone. An oestradiol drawn at peak on a large infrequent injection will look alarming and does not represent your average exposure.

Interpret symptoms, not numbers:

  • High: water retention, nipple sensitivity or gynecomastia, emotional lability.
  • Low: joint pain and clicking, loss of libido despite adequate testosterone, low mood, poor sleep, dry skin. Low oestradiol symptoms are frequently misattributed to insufficient testosterone, leading to a dose increase that makes the underlying problem worse.

If starting or increasing an aromatase inhibitor: recheck oestradiol and lipids within 8 weeks. The lipid consequence is documented and is not theoretical.

Management Strategies

Change the dosing pattern before reaching for a drug:

  • Splitting the weekly dose into two or three smaller injections lowers the peak and usually resolves peak-driven oestrogenic symptoms without any aromatase inhibitor at all.

If an aromatase inhibitor is genuinely needed:

  • Use the smallest effective dose, titrate slowly, and recheck both oestradiol and lipids.
  • Do not target a specific number. Target symptom control.
  • Anastrozole at typical TRT doses can overshoot easily, and the joint pain and libido loss from over-suppression are worse than the symptoms most men are treating.

Body composition is a real lever: aromatase is expressed in adipose tissue, so reducing body fat reduces conversion at the same testosterone dose.

Do not switch esters for oestrogen control: cypionate and enanthate aromatise identically. Frequency and dose are what change the result.

Oestradiol management is the most nuanced part of testosterone therapy and the area where the most avoidable harm is done. Adequate oestradiol is required for bone mineral density, joint health, libido, mood, cognition and hepatic LDL receptor activity, and Zmuda (1993, PMID 8487666) showed experimentally that blocking aromatisation on a testosterone protocol worsened HDL, apoA-I and hepatic lipase across the board. Nothing about the cypionate ester changes aromatisation relative to enanthate; the practical levers are weekly dose, injection frequency and body composition. Men who chase a target oestradiol number with an aromatase inhibitor rather than treating symptoms routinely end up with joint pain, low libido and a worse lipid panel than they started with.
