How Testosterone Cypionate Affects LDL Cholesterol

Injectable testosterone does not reliably raise LDL, and cypionate behaves exactly as enanthate does. Controlled trials show LDL unchanged or modestly lower. When LDL climbs on a cycle, an oral compound or an over-suppressed oestradiol is nearly always responsible.

The Mechanism

The cypionate ester is cleaved before the hormone reaches the liver, so the LDL mechanism is identical to that of testosterone enanthate:

  1. Route of administration is the determining factor: Thompson (1989, PMID 2915439) tested whether the lipid damage from anabolic steroids is driven by route or by androgenic potency, giving the same 11 weightlifters oral stanozolol and intramuscular testosterone enanthate in a crossover design. Oral stanozolol raised LDL 29% and hepatic lipase 123%; injectable testosterone lowered LDL 16% and produced a hepatic lipase change that was not significant. An injected ester never delivers the concentrated first-pass hepatic load that drives LDL receptor downregulation.

  2. Aromatisation supports LDL clearance: Oestradiol upregulates hepatic LDL receptor expression. Testosterone aromatises, so it partly funds its own LDL protection. Zmuda (1993, PMID 8487666) showed that blocking aromatisation worsened every lipid endpoint measured.

  3. ApoB can rise while LDL cholesterol does not: Hartgens (2004, PMID 15155420) measured apolipoproteins in strength athletes on self-administered stacks and found total cholesterol unchanged while apoB rose from 0.96 to 1.32 g/L. LDL cholesterol measures cholesterol mass; apoB counts atherogenic particles. A normal LDL cholesterol does not rule out an increase in particle number.

  4. Lipoprotein(a) falls: Androgens lower Lp(a), a largely diet-resistant and genetically determined risk factor.

Expected Changes

Replacement doses (100-200 mg/week):

  • Whitsel (2001, PMID 11566455) pooled 19 studies covering 272 hypogonadal men on an average of 179 mg of intramuscular testosterone ester every 16 days for 6 months: LDL fell 5 mg/dL (95% CI -8 to -1), total cholesterol fell 14 mg/dL, HDL fell 4 mg/dL.
  • In practice most men on TRT see LDL within a few mg/dL of baseline.

200 mg/week in trained men (Thompson 1989, PMID 2915439):

  • LDL fell 16% over six weeks.

Supraphysiological doses (300-600 mg/week):

  • Bhasin (2001, PMID 11701431) gave graded doses to 600 mg/week for 20 weeks and reported HDL falling in a clear dose-dependent manner; LDL was not among the markers reported to change significantly.

Testosterone plus orals (Webb 1984, PMID 6493049):

  • LDL of 150 mg/dL on cycle versus 125 mg/dL off in bodybuilders using 50-100 mg/day methandrostenolone alongside testosterone and nandrolone, with an LDL to HDL ratio of 6.0 versus 2.2.
  • The oral is the driver in that picture, not the testosterone.

With an aggressive aromatase inhibitor: expect LDL to move the wrong way, because you have removed the hepatic LDL receptor support.

Timing: Slow and modest. Check at 8 to 12 weeks after a change, not at week 2.

Monitoring Guidance

Baseline: Full lipid panel and ApoB if your lab offers it.

On protocol:

  • Recheck 8 to 12 weeks after starting or after a dose change.
  • Annually once stable.
  • Recheck within 8 weeks of starting or increasing an aromatase inhibitor.

Believe ApoB over LDL cholesterol when they disagree: Hartgens documented exactly the pattern of unchanged cholesterol mass with sharply increased particle count on androgens. Non-HDL cholesterol (total minus HDL) is a free substitute if ApoB is unavailable.

Attribution: if LDL rose after you added an oral, the oral did it. Removing the oral is the intervention that works, and there is trial evidence on both sides of that statement.

Management Strategies

Do not crush oestradiol: it is the most common avoidable cause of a deteriorating lipid panel on testosterone, and the mechanism has been demonstrated experimentally.

Testosterone is not the compound to drop for LDL reasons: the oral 17-alpha-alkylated compounds in the stack are.

Standard lipid management works normally on TRT:

  • Statins, ezetimibe and dietary changes behave the same in men on testosterone as in anyone else.
  • Soluble fibre 10 to 25 g/day, replacing saturated with unsaturated fat, and 150 minutes per week of moderate cardio are all first-line and unaffected by the testosterone.

Wider context: Baggish (2017, PMID 28533317) compared 86 long-term anabolic steroid users with 54 non-using weightlifters and found greater coronary plaque volume in users, with lifetime dose strongly associated with atherosclerotic burden. A benign LDL is reassuring for one marker, not for the whole cardiovascular picture of long-term supraphysiological use.

Clinical Significance

Testosterone cypionate, like testosterone enanthate, is the least LDL-hostile anabolic compound available, supported by unusually good evidence: a randomised crossover trial showing LDL down 16% and a meta-analysis of 272 hypogonadal men showing LDL down 5 mg/dL. The clinically useful conclusion is attributional. A man on testosterone whose LDL climbs is almost always seeing the effect of an oral 17-alpha-alkylated compound or an aromatase inhibitor that has removed oestradiol's support for hepatic LDL receptor activity. The second conclusion is that a normal LDL cholesterol on androgens does not prove a normal atherogenic particle burden, because apoB rises on stacks while total cholesterol stays put.
