How Testosterone Cypionate Affects SHBG
Testosterone cypionate suppresses hepatic SHBG production within 1-2 weeks, raising the free testosterone fraction. The effect is dose-dependent and identical to enanthate. Its main importance is interpretive: total testosterone cannot be read sensibly without knowing SHBG.
The Mechanism
Sex hormone binding globulin is produced by the liver and binds circulating testosterone with high affinity, leaving only a small unbound fraction biologically active. Exogenous androgens suppress hepatic SHBG synthesis, and testosterone cypionate does this identically to enanthate because the ester is cleaved before the hormone reaches the hepatocyte.

Direct hepatic suppression: Androgen receptor signalling in the liver downregulates SHBG gene transcription. The effect is dose-dependent and appears within 1 to 2 weeks.

Insulin and thyroid modulate it too: Insulin suppresses SHBG independently, which is why men with insulin resistance often present with low SHBG before any androgen exposure. Thyroid hormone raises it. A very low SHBG on TRT may reflect metabolic health as much as the testosterone dose.

The free fraction rises disproportionately: When SHBG falls, a larger share of total testosterone is unbound. Two men with identical total testosterone and different SHBG have meaningfully different free testosterone and will feel different.

Oral 17-alpha-alkylated steroids suppress it far harder: Orals reach the liver in concentration and can drive SHBG to near-undetectable levels within days, which is why total testosterone readings on oral-containing stacks are close to uninterpretable without free testosterone.
Cypionate versus enanthate: no difference in SHBG suppression. Injection frequency has a modest effect through peak concentration.
Expected Changes
Replacement doses (100-200 mg/week):
- SHBG typically falls 30 to 50% from baseline within the first 4 to 8 weeks
- A pre-TRT SHBG of 35 nmol/L commonly settles between 15 and 25 nmol/L
- Free testosterone rises proportionally more than total testosterone
Supraphysiological doses (300-600+ mg/week):
- SHBG can fall below 10 nmol/L
- At this point total testosterone substantially understates biological androgen exposure
With oral 17-alpha-alkylated compounds added:
- SHBG can fall below 5 nmol/L, sometimes to the limit of assay detection
- Oxandrolone in particular is documented to suppress SHBG aggressively
Timing: Falls within 1 to 2 weeks, stabilises by week 4 to 8. Recovers over 4 to 8 weeks after stopping, though recovery is slower after prolonged suppression.
Directionality note: SHBG suppression is not an adverse effect in itself. It is the mechanism by which a modest total testosterone can produce a strong androgenic effect.
Monitoring Guidance
Always order SHBG alongside total testosterone. A total testosterone in isolation cannot be interpreted, and this is the single most common bloodwork mistake in men on TRT.
Baseline: SHBG before starting, so the degree of suppression can be quantified later.
On protocol:
- Check at 6 to 8 weeks alongside total testosterone and, ideally, free testosterone.
- Free testosterone by equilibrium dialysis is the reference method. Calculated free testosterone from total, SHBG and albumin is acceptable and far cheaper.
Interpreting a low SHBG:
- Expected on testosterone and generally benign.
- A very low SHBG with a high free testosterone and symptoms of androgen excess is a reason to reduce dose, not to chase the SHBG number.
- A low SHBG that predated TRT points at insulin resistance and is worth investigating with fasting insulin, HbA1c and a lipid panel.
Interpreting a high SHBG: uncommon on testosterone. Consider thyroid status, liver disease, or an oestradiol that is running high.
Management Strategies
Do not treat SHBG as a target to fix. It is a downstream marker, and there is no intervention that raises it on TRT without reducing the androgen exposure that is suppressing it.
Use it for interpretation:
- Low SHBG plus normal total testosterone plus good free testosterone plus no symptoms equals a working protocol.
- Low SHBG plus normal total testosterone plus symptoms of androgen excess suggests reducing the dose.
Injection frequency: More frequent smaller injections produce lower peak concentrations and slightly less aggressive SHBG suppression, though the effect is smaller than the dose effect.
Address the metabolic contributors: Insulin resistance suppresses SHBG independently. Improving insulin sensitivity through body composition, training and diet raises SHBG through a pathway unrelated to the testosterone.
Drop the orals if SHBG is on the floor: An SHBG below 5 nmol/L on a stack means the oral is doing most of the work, and total testosterone readings become close to meaningless.

Clinical Significance
SHBG suppression on testosterone is expected, dose-dependent and usually benign in itself. Its importance is interpretive: without SHBG, a total testosterone result cannot be converted into any statement about how much bioavailable androgen a man is actually exposed to, and two men with the same total testosterone and different SHBG can have very different clinical pictures. Nothing about the cypionate ester alters this relative to enanthate. The two practical consequences are that SHBG should always be ordered alongside total testosterone, and that a very low SHBG which predated testosterone therapy is a marker of insulin resistance worth investigating on its own terms.
