When Will Orforglipron Be Available in Australia? (TGA Update)

If you saw this week's headline about a weight-loss pill being approved in Australia, you probably assumed it was orforglipron. It wasn't. On 3 November 2026 the TGA approved the Wegovy Pill, which is oral semaglutide from Novo Nordisk. Eli Lilly's orforglipron, sold in the US as Foundayo, is still waiting.
That difference matters. The two pills are different drugs, taken differently, with different side effects. Below: where orforglipron is up to in Australia, when it will realistically arrive, what the trials showed, how much of the weight you lose is muscle, and the bloodwork worth running if you end up on it.
This article is educational, not medical advice. Orforglipron is not registered in Australia. Talk to your GP or endocrinologist about weight-loss medication, and do not take any drug sourced outside the regulated supply chain.
Quick answer: Orforglipron (Foundayo) is not TGA approved as of 6 November 2026. The pill the TGA approved on 3 November is the Wegovy Pill (oral semaglutide 25mg), and orforglipron was reported as "also awaiting TGA approval". Lilly said it would apply by the end of 2025, and the application was reported as under review in May 2026. On the TGA's standard 255 working-day clock, a decision in late 2026 or the first half of 2027 is plausible, with launch after that and no PBS subsidy for weight loss in sight. The FDA approved it on 1 April 2026 and the UK on 10 August 2026. In trials it took off 11.2% of body weight at 72 weeks on the top dose, raised resting heart rate by about 4 to 5 bpm, and about 27% of the weight lost was lean mass. Timeline, trial data and a monitoring schedule are below.
Is orforglipron TGA approved?
No. As of 6 November 2026 there is no orforglipron or Foundayo entry on the Australian Register of Therapeutic Goods (ARTG), and nothing dispensed in an Australian pharmacy under either name.
What did get approved is the Wegovy Pill. It entered the ARTG on 3 November 2026 as the first oral GLP-1 approved by the TGA for weight management, for adults with a BMI of 30 or more, or 27 or more with a weight-related condition, and for cardiovascular risk reduction in people with established heart disease (Australian Pharmacist, 2026). The same article notes that "Eli Lilly's oral GLP-1 RA orforglipron (Foundayo) is also awaiting TGA approval." The Wegovy Pill had no launch date or price at approval, and the TGA still needed to review packaging and production before release.
Lilly told Australian media in 2025 that it would submit orforglipron to the TGA for both obesity and type 2 diabetes by the end of that year (Pharmacy Daily), and by May 2026 the TGA was reported to be reviewing the application (The Nightly, 2026). Neither Lilly nor the TGA has published a decision date.
Wegovy Pill vs orforglipron: not the same drug
| Wegovy Pill | Orforglipron (Foundayo) | |
|---|---|---|
| Company | Novo Nordisk | Eli Lilly |
| What it is | Semaglutide, a peptide, in a tablet with an absorption enhancer | A small-molecule, non-peptide drug |
| How you take it | On waking, empty stomach, no more than 120mL of water, then 30 minutes before food, drink or other tablets | Any time of day, with or without food or water |
| Top dose | 25mg daily | 17.2mg daily |
| Weight loss in its main trial | 13.6% at 64 weeks (OASIS 4) | 11.2% at 72 weeks (ATTAIN-1) |
| TGA status (6 Nov 2026) | Approved 3 Nov 2026 | Under evaluation |
The Wegovy Pill figure comes from OASIS 4, where oral semaglutide 25mg gave 13.6% weight loss versus 2.2% on placebo (Wharton et al., 2025). A Novo-funded indirect comparison put oral semaglutide about 3 percentage points ahead of orforglipron for weight loss, with fewer GI dropouts (Michalak et al., 2026). Take that one with salt: the company selling the competitor paid for it, and it compares across trials.
The fasting rule is the real day-to-day difference. Semaglutide is a peptide and gets digested, so the tablet only works on an empty stomach. Orforglipron is a small chemical molecule that survives the gut, so it needs no fasting window.
When will Foundayo be available in Australia?
Nobody outside Lilly and the TGA knows the date, and neither has published one. Here is how to estimate it.
The TGA's legislated target for a standard new medicine is 255 working days. Priority review is 150 working days, and the comparable overseas regulator (COR) pathways, which lean on an FDA or MHRA assessment, are 120 or 175 working days. Those clocks pause every time the TGA sends questions and waits for the sponsor's answers, so the calendar time is longer than the working days suggest.
If Lilly lodged near the end of 2025 on the standard pathway, roughly 12 months of TGA clock time puts a decision somewhere between late 2026 and the first half of 2027. If Lilly used a COR pathway off the back of the April FDA approval, it could be sooner. Lilly has not said which pathway it used.
Approval doesn't mean it's on the shelf. Injectable Wegovy had its TGA decision in January 2024 and ARTG entry in February 2024 (TGA AusPAR, 2024), and supply arrived months later. The Wegovy Pill is repeating the pattern: approved, no launch date.
| Regulator | Orforglipron status |
|---|---|
| US FDA | Approved 1 April 2026 (Foundayo, weight management) |
| UK MHRA | Approved 10 August 2026 (weight management and type 2 diabetes) |
| Australia TGA | Under evaluation, no public decision date |
| EU and others | Not confirmed |
The FDA approval went through the Commissioner's National Priority Voucher pilot (Pharmacy Times, 2026). The MHRA described itself the first regulator in Europe to authorise it (Pharmaceutical Journal, 2026).
Will it be on the PBS?
Not for weight loss any time soon. GLP-1 drugs on the PBS are subsidised for type 2 diabetes only. Injectable Wegovy's PBS case has been limited to people with obesity and established cardiovascular disease, and a revised submission is on the November 2026 PBAC agenda (PBS). There is no PBAC submission for orforglipron, and a PBS listing would need TGA registration first. Expect to pay privately.
How much will it cost?
There is no Australian price. In the US, Lilly sells Foundayo self-pay through LillyDirect from $149 a month at the lowest dose, with higher doses costing more. Private script prices in Australia usually land well above US cash prices once a drug is registered. Budget for the cost of a private Mounjaro or Wegovy script, not the US figure.
What orforglipron actually is
Orforglipron started life as OWL833 at Chugai and became LY3502970 at Lilly. It is the first non-peptide GLP-1 receptor agonist on the market. It binds a pocket at the top of the GLP-1 receptor that the peptide drugs do not use, and it is a partial agonist that favours G-protein signalling over beta-arrestin recruitment (Kawai et al., 2020).
Its half-life is about 29 to 49 hours on the label, so it reaches steady state in about a week, which suits once-daily dosing. In the first human study, single doses lasted 25 to 35 hours and repeat dosing stretched that to 48 to 68 hours (Pratt et al., 2023).
Dosing on the US label
| Step | Tablet | Minimum time on the dose |
|---|---|---|
| 1 | 0.8mg | 30 days |
| 2 | 2.5mg | 30 days |
| 3 | 5.5mg | 30 days |
| 4 to 6 | 9mg, 14.5mg, 17.2mg | 30 days each, if needed |
The maximum is 17.2mg a day (Lilly prescribing information). If you miss seven or more days in a row, you restart lower on the ladder.
One trap. The trials used 6, 12 and 36mg capsules, which correspond to the 5.5, 9 and 17.2mg tablets. When a paper says "36mg", it means the top approved tablet, not some bigger dose.
The CYP3A4 rule most people miss
The liver enzyme CYP3A4 clears most of the drug. Clarithromycin, a common antibiotic, raised orforglipron levels 3.5-fold, and the anticonvulsant carbamazepine cut them by 82% (Lilly prescribing information; Morse et al., 2026). On the label:
- With a strong CYP3A4 inhibitor, the dose is capped at 9mg. Avoid ritonavir-type drugs entirely.
- Avoid strong inducers such as carbamazepine.
- Simvastatin is capped at 20mg a day, because orforglipron doubles simvastatin exposure.
- Women on the oral contraceptive pill need a backup method for 30 days after starting and after every dose increase.
Testosterone esters, nandrolone, aromatase inhibitors, clomiphene and tamoxifen are not known strong CYP3A4 inhibitors or inducers. The real risk for lifters is a short course of clarithromycin or an azole antifungal for a skin infection while you are on the top dose.
How much weight does orforglipron take off?
In ATTAIN-1, 3,127 adults with obesity and no diabetes took orforglipron or placebo for 72 weeks. Counting everyone who was randomised, average weight loss was 7.5%, 8.4% and 11.2% at the three doses, versus 2.1% on placebo (Wharton et al., 2025). On the top dose, 54.6% lost at least 10%, 36.0% lost at least 15%, and 18.4% lost at least 20%. If you only count people who stayed on the drug, the top dose averaged 12.4%.
With type 2 diabetes the effect is smaller, as with every GLP-1: 9.6% at the top dose in ATTAIN-2 (Horn et al., 2026).
Orforglipron vs Zepbound and Wegovy injections
There is no head-to-head trial, so these numbers come from different trials with different lengths and populations:
| Drug | Trial | Length | Average weight loss |
|---|---|---|---|
| Orforglipron 17.2mg | ATTAIN-1 | 72 weeks | 11.2% |
| Oral semaglutide 25mg (Wegovy Pill) | OASIS 4 | 64 weeks | 13.6% |
| Semaglutide 2.4mg injection (Wegovy) | STEP 1 | 68 weeks | 14.9% |
| Tirzepatide 15mg injection (Mounjaro, Zepbound) | SURMOUNT-1 | 72 weeks | 20.9% |
Sources: Wilding et al., 2021; Jastreboff et al., 2022.
So is orforglipron better than Zepbound? Not for weight loss. Tirzepatide takes off nearly twice as much. Orforglipron's pitch is that it is a pill with no fasting rules, and it should be cheaper to make at scale.
Against oral semaglutide in type 2 diabetes
ACHIEVE-3 put orforglipron against Rybelsus in 1,698 people with type 2 diabetes on metformin for 52 weeks. Orforglipron lowered HbA1c by 1.71 and 1.91 points at its two doses, versus 1.23 and 1.47 for semaglutide 7 and 14mg, and won on both (Rosenstock et al., 2026). The price was tolerability: GI side effects in 58 to 59% versus 37 to 45%, and about twice as many people stopping because of side effects.
Switching from injections to the pill
ATTAIN-MAINTAIN took people who had already lost weight on tirzepatide or semaglutide injections and switched them to orforglipron or placebo for a year. Among people whose weight had plateaued, after tirzepatide orforglipron users kept 74.7% of their lost weight versus 49.2% on placebo. After semaglutide, it was 79.3% versus 37.6% (Aronne et al., 2026).
That helps if you want off weekly injections. It doesn't show the pill is as good as staying on the injection, because nobody in the trial stayed on theirs.

How much muscle do you lose on orforglipron?
In a body-composition (DXA) subgroup of 171 people from ATTAIN-1, fat mass fell 9.4% to 20.0% and visceral fat 12.6% to 28.2% across the doses, while lean mass fell 3.2% to 6.6% (Lilly Medical Information). Pooled across doses, 26.9% of the weight lost was lean mass and 73.1% was fat.
For context, a 2026 meta-analysis of 20 trials found the lean share of weight lost was 35.2% on semaglutide, 25.4% on tirzepatide, 26.2% with diet and lifestyle alone, and 17.5% when lifestyle included resistance training (Eisa and Barood, 2026). So orforglipron lands right where dieting lands. Training moves that number more than the drug does.
Two caveats. These were people with obesity who weren't lifting, not trained men on TRT. And DXA "lean mass" includes water and glycogen, so part of the early drop isn't muscle. Neither caveat makes the number go away.
What if I lose 10kg on orforglipron?
Run the numbers. At the trial split, about 2.7kg of a 10kg loss would be lean mass. At the resistance-training split of 17.5%, it would be about 1.75kg. That kilogram is the gap between keeping your training and letting it slide while your appetite vanishes. Keep lifting two to three times a week and keep protein around 1.6g per kg of body weight. That protein target is expert consensus rather than trial-proven in GLP-1 users, but it is the one lifters already use. More in how much muscle you lose on retatrutide.
Bloodwork to run on orforglipron
The label doesn't mandate routine bloodwork. These are the markers where the trial data, the warnings or the drug's history give you a reason to look.
Liver: why everyone asked
Orforglipron's two closest rivals both ended on liver signals. Pfizer stopped lotiglipron after transaminase rises in about 6% of people taking it, versus 1.6% on placebo in the obesity cohort (Amin et al., 2025). It then dropped danuglipron in April 2025 after a possible drug-induced liver injury in one trial participant, which resolved when the drug was stopped. So orforglipron got watched closely.
The data reassure. A pooled analysis of 11,220 people across seven phase 3 trials found ALT and AST fell on average, and combined ALT or AST above three times the upper limit with bilirubin above twice the limit happened in 0.1% on orforglipron and 0.1% on comparators, all with other explanations (Wharton et al., 2026). No drug-induced liver injury turned up.
The catch for lifters: if you run oral steroids, your ALT is already noisy, and the orforglipron obesity trials excluded people starting with ALT above three times normal. Get a baseline before you start so you can tell which drug moved it. Add GGT, which training doesn't raise, to separate liver from muscle.
Pancreas: expect lipase to rise
On the label, lipase rose 26 to 31% and pancreatic amylase 16 to 20% on average, against 3 to 4% on placebo, without symptoms. Pancreatitis is a label warning, as it is for every GLP-1.
A lipase above range with no pain isn't a reason to stop. Severe upper abdominal pain that goes through to your back, with or without vomiting, is. Stop and get seen the same day.
Kidneys: the dehydration risk
Acute kidney injury after vomiting or diarrhoea is a label warning. Check creatinine and eGFR at baseline and after any bad GI stretch. If you carry a lot of muscle, creatinine makes your kidneys look worse than they are, so add cystatin C (why muscular athletes get false kidney readings). Dehydration also concentrates your blood, so draw haematocrit well hydrated if you are on TRT.
Lipids and glucose
At the top dose in ATTAIN-1, triglycerides fell about 17% more than placebo, HDL rose about 5%, and LDL barely moved. Systolic blood pressure fell about 5 mmHg. If steroids have crushed your HDL, this is a small offset, not a fix. Without diabetes, HbA1c fell only 0.3 points, because there was little to lower.
Heart rate: the one to watch on cycle
Across the obesity trials, orforglipron raised resting heart rate by 4 to 5 bpm versus 0.5 bpm on placebo, and 3% had tachycardia-type side effects versus 0.9% on placebo (Lilly prescribing information). A 2026 network meta-analysis found orforglipron raised heart rate more than any other drug in the class, about 7 bpm in the pairwise analysis and about 9 bpm for the top trial dose in the network analysis (with a wide range, 4.5 to 13.9), against about 3.5 bpm for retatrutide and 2 bpm for tirzepatide (Zhang et al., 2026).
Now stack it up. Clenbuterol, T3, trenbolone and pre-workout stimulants all push resting heart rate up too. Orforglipron on top of a cutting stack can add 10 to 20 bpm before you notice. Log your morning resting heart rate for a week before you start so you have a baseline, and read why your zone 2 heart rate is wrong on clen, T3 and tren before you trust your watch.
Monitoring schedule
| When | What to run |
|---|---|
| Baseline | ALT, AST, GGT, bilirubin, creatinine, eGFR (plus cystatin C if muscular), lipid panel, HbA1c, fasting glucose, full blood count, lipase if you drink heavily or have had pancreatitis, 7-day resting heart rate and blood pressure average. Add testosterone, estradiol and haematocrit if on TRT |
| Week 4 to 6 | ALT, AST, creatinine and eGFR, haematocrit if on TRT, resting heart rate trend |
| Each dose step | Resting heart rate and blood pressure; labs only if you have symptoms or an earlier abnormal result |
| Month 3, then every 3 months | Repeat the baseline panel, plus a body-composition or strength check |
| Any time | Severe abdominal pain: lipase and medical review. Persistent vomiting or diarrhoea: creatinine and electrolytes. New antibiotic or antifungal: check the CYP3A4 rule |
This schedule is our suggestion, not a label requirement. For the wider GLP-1 monitoring logic, see the GLP-1 bloodwork guide for semaglutide and tirzepatide and the retatrutide bloodwork guide.

Can I get orforglipron in Australia now?
Not legally through normal channels. Here is how the options sort out.
- Do you want a GLP-1 now? Mounjaro (tirzepatide) and injectable Wegovy are registered and can be prescribed privately. Both out-perform orforglipron for weight loss.
- Do you specifically need a pill? The Wegovy Pill is approved but not launched. When it arrives, it is the registered oral option, if you can live with the fasting rule.
- Thinking of importing Foundayo? The TGA's Personal Importation Scheme lets you import up to three months of a prescription medicine at a time, but only with a valid Australian prescription, and only for products legitimately made overseas (TGA). In practice, Foundayo is dispensed in the US against US prescriptions, and an Australian doctor would need a good reason to prescribe an unregistered drug when registered alternatives exist.
- Looking at "research" orforglipron? Peptide shops sell orforglipron powder and capsules labelled "research use only, not for human consumption". Nobody has published Australian testing of these products, so nobody can tell you the dose or purity. The Personal Importation Scheme does not cover research chemicals, and imports that do not comply can be seized.
Grey-market orforglipron carries an extra risk. A bad batch of a peptide like semaglutide is often just weak. A small molecule can be made cheaply at any purity, and you can't check what you have without a lab test. The two Pfizer drugs above show liver toxicity is a real risk in this chemical class, and a bootleg version made without proper quality control is where it would show up.
The TGA also bans advertising of GLP-1 medicines to the public (TGA). Any Australian site advertising orforglipron to you is already breaking the rules.
Practical recommendations
- Waiting for orforglipron? Watch the TGA's prescription medicines under evaluation list and the ARTG. Expect a decision in late 2026 or 2027, launch some months after, and no PBS subsidy for weight loss.
- Want to start now? Tirzepatide or semaglutide on a private script are the registered routes, and both take off more weight than orforglipron.
- Prefer a pill? Compare the Wegovy Pill's fasting rule with your morning routine, especially if you take thyroid hormone or oral compounds on waking.
- On TRT or a cycle? Get a baseline panel, log resting heart rate, and count orforglipron's 4 to 9 bpm rise as one more item on your stimulant budget.
- Protect your muscle. Lift two to three times a week and eat about 1.6g of protein per kg. That does more for lean-mass loss than choosing between GLP-1 drugs.
- Peptide stackers: if you already run MK-677 or tesamorelin, watch fasting glucose and HbA1c when you add any GLP-1, so you can see which drug is doing what.

Try it FreeKey takeaways
- Orforglipron (Foundayo) is not TGA approved as of 6 November 2026. The pill approved on 3 November is the Wegovy Pill, which is oral semaglutide.
- Lilly applied around the end of 2025. A decision in late 2026 or the first half of 2027 is plausible, launch comes later, and there is no PBS subsidy for weight loss.
- The FDA approved it on 1 April 2026 and the UK MHRA on 10 August 2026.
- Average weight loss was 11.2% at 72 weeks on the top dose: less than Wegovy injections or tirzepatide, but with no fasting rule.
- About 27% of the weight lost was lean mass, the same as dieting. Resistance training cuts that more than switching drugs does.
- Resting heart rate rises 4 to 5 bpm on the label and up to 9 bpm at the top trial dose. It stacks with clen, T3 and tren.
- Liver data from 11,220 people are clean, lipase rises without symptoms, and dehydration can hit the kidneys. Get a baseline panel.
- Strong CYP3A4 inhibitors such as clarithromycin cap the dose at 9mg.
