How Superdrol Affects LDL Cholesterol
Superdrol (methasterone) has no controlled human lipid trial of any kind. What exists is a case report literature of severe cholestatic liver injury from the designer supplement era. Every structural feature that drives LDL up in the oral steroid class is present, without the aromatisation that partly protects dianabol, so expect a severe LDL rise you cannot quantify.
The Mechanism
Methasterone is a 17-alpha-alkylated, non-aromatising methylated dihydrotestosterone derivative. Every one of those descriptors points the same way for LDL:

17-alpha-alkylation drives first-pass hepatic loading: This is the structural feature Thompson (1989, PMID 2915439) identified as the cause of LDL elevation in anabolic steroids. Oral stanozolol raised LDL 29% while injectable testosterone in the same men lowered it 16%, and the difference was route, not androgenic potency. Methasterone shares the structure.

No aromatisation and no oestrogenic activity: Methasterone is a DHT derivative and does not convert to oestradiol. Oestradiol is what upregulates hepatic LDL receptors, so there is no counterbalance to androgenic receptor downregulation. Dianabol at least aromatises; superdrol does not.

Marked hepatotoxicity indicates a heavy hepatic signal: Methasterone was sold as a dietary supplement until it was scheduled, and the case report literature that followed documents cholestatic hepatitis and drug-induced liver injury (Abeles 2020, PMID 33214235 describes two anabolic steroid-induced liver injury cases assessed formally for causality; Tran et al. (2023, PMID 36796473) catalogued androgenic steroids found in over-the-counter supplements and their associated adverse events). A compound producing cholestatic injury is disturbing hepatic bile acid and lipoprotein handling at the same time.

Cholestasis affects lipids directly: Cholestatic liver injury can itself distort the lipid panel, including the appearance of lipoprotein X, which some assays report as LDL. On a superdrol cycle with a rising bilirubin, an abnormal LDL result may reflect cholestasis as much as steroid-driven receptor downregulation.
What is genuinely unknown: nobody has given methasterone to human subjects in a controlled setting and measured LDL. The mechanism is well characterised for the class; the magnitude for this specific compound is not.
Expected Changes
There is no measured figure for methasterone. No randomised trial, no crossover study, no cohort with pre and post lipids. Any specific percentage attributed to superdrol online is invented.
What the class-level evidence supports:
- Glazer (1991, PMID 1929679) pooled the anabolic steroid literature and found a weighted average LDL rise of 36%.
- Thompson (1989, PMID 2915439) measured a 29% LDL rise from stanozolol at 6 mg/day, a non-aromatising 17-alpha-alkylated oral with the closest structural profile to methasterone of any compound with real data.
- Webb (1984, PMID 6493049) found an LDL to HDL ratio of 6.0 in bodybuilders on oral-containing stacks versus 2.2 off.
Practical expectation at typical doses (10-20 mg/day):
- LDL up by at least the class average of 36%, plausibly more given the absence of any oestrogenic offset.
- HDL collapse into single figures or the low teens is commonly reported in user bloodwork, which is consistent with the mechanism though not with any trial.
- The LDL to HDL ratio is the number that will look worst.
Timing: Superdrol acts fast on every hepatic endpoint. Assume lipid changes are established by week 2 and near maximal by week 3.
Recovery: Reversible on stopping, over roughly 8 to 12 weeks, provided liver function recovers. Cholestatic injury can take considerably longer.
Monitoring Guidance
Baseline lipid panel and full liver panel including bilirubin and ALP: With superdrol the cholestatic picture and the lipid picture are entangled, and you cannot interpret one without the other.
On cycle:
- Check at week 2, not week 4. Superdrol runs are typically 3 to 4 weeks and by week 4 the decision point has passed.
- Include bilirubin and alkaline phosphatase. A rising bilirubin with jaundice or dark urine ends the cycle immediately, regardless of what the lipids say.
- If bilirubin and ALP are elevated, treat any LDL result with caution. Cholestasis can produce lipoprotein X, which some LDL assays cannot distinguish from genuine LDL.
Track the ratio: LDL to HDL and non-HDL cholesterol are more informative than LDL alone when both sides are moving hard.
Post-cycle: Recheck lipids and liver panel at 4 and 12 weeks. If liver enzymes have not normalised by 12 weeks, see a doctor rather than starting another cycle.
Management Strategies
Duration is the only real control:
- Keep runs to 3 to 4 weeks. Superdrol produces its adverse effects quickly, and extending the run compounds hepatic and lipid damage in parallel.
- Never stack superdrol with another 17-alpha-alkylated oral. The hepatotoxicity is additive and superdrol already sits at the top of the range.
Supportive measures, honestly framed:
- Cardiovascular exercise 30 to 45 minutes, 4 to 5 times per week is the intervention with the best evidence, and it will still be overwhelmed by the compound.
- Soluble fibre 10 to 25 g/day for LDL via bile acid sequestration.
- TUDCA is used widely for cholestasis on superdrol and has plausible mechanistic support for the bile acid problem. It does nothing for LDL and it is not a reason to run the compound longer.
- No supplement stack has been shown to prevent the lipid damage from a non-aromatising 17-alpha-alkylated oral.
The realistic option: If the goal is a short strength or hardness block and the lipid cost matters to you, the compounds with actual human data and less extreme profiles are the better choice. Superdrol asks you to accept an unmeasured risk.

Clinical Significance
Superdrol is the clearest example in this catalogue of a compound where the mechanism is certain and the magnitude is unknown. Methasterone is a 17-alpha-alkylated, non-aromatising oral, which is precisely the structural combination Thompson (1989, PMID 2915439) showed drives LDL up and HDL down simultaneously, and it carries the additional problem that its documented cholestatic liver injury can distort the lipid panel independently of any receptor-level effect. No controlled trial has ever measured lipids on methasterone in humans, so any specific number quoted for it is fabricated. The practical implication is that superdrol should be monitored more aggressively than compounds with published data, not less, because you cannot predict where your own numbers will land.
