The share of abnormal sperm with mid-piece defects. The mid-piece houses the mitochondria that power movement.
PED Notes
No reference range, for the same reason as the other morphology sub-types: it is a breakdown of the abnormal forms, not a pass or fail. Its interest is mechanistic. The mid-piece contains the mitochondria, so mid-piece defects tend to travel with poor motility and poor velocity, and both respond to the same mitochondrial support. Excess residual cytoplasm is usually reported nearby and is the more informative of the two.
Descriptive rather than diagnostic. Use Sperm Morphology for the assessment.
Why it tracks with motility: the mid-piece holds the mitochondria that generate the energy for movement, so mid-piece defects commonly appear alongside low Total Motility and low Average Sperm Velocity.
Mitochondrial support is the relevant angle: CoQ10 and L-carnitine have the best evidence, dosed as described under Sperm DNA Fragmentation. Allow 3 months.
Check Excess Residual Cytoplasm on the same report, which is the more informative marker of immature sperm and oxidative stress.
