The most informative of the morphology sub-types and the one most worth understanding. Retained cytoplasm is not merely cosmetic: it is loaded with enzymes that generate reactive oxygen species, so these sperm actively produce oxidative stress inside the sample rather than just tolerating it. That makes excess residual cytoplasm a mechanistic link to DNA fragmentation, and a plausible explanation when fragmentation is high while count and motility look acceptable. It reflects spermatogenesis being rushed or disrupted, which is exactly what suppression, heat and varicocele do.

When high

Why this one matters more than the other morphology sub-types: retained cytoplasm carries enzymes that actively generate reactive oxygen species. These sperm are a source of oxidative stress, not just a casualty of it, which is why raised ERC and raised DNA fragmentation so often appear together.

Check Sperm DNA Fragmentation if it has not been done. Raised ERC is one of the better reasons to order it, particularly when count and motility look acceptable.

What it reflects: spermatogenesis being rushed or disrupted, so sperm are released before maturation completes. Common drivers are varicocele, scrotal heat, suppressed intratesticular testosterone, infection and oxidative load.

Practical steps:

  • Rule out varicocele with a scrotal ultrasound if this is persistently high
  • Remove heat exposure: saunas, hot baths, prolonged cycling, laptops
  • Antioxidant support as described under Sperm DNA Fragmentation
  • Allow a full 3 months before retesting
