How Testosterone Cypionate Affects Creatinine

Testosterone cypionate raises serum creatinine by roughly 5-15% on replacement doses, mostly because creatinine is produced by muscle and testosterone builds muscle. The rise is usually artefact rather than kidney injury, but creatinine alone cannot tell the difference. Cystatin C and urine ACR can.

The Mechanism

Creatinine is the breakdown product of creatine phosphate in skeletal muscle, produced at a rate roughly proportional to muscle mass and cleared by the kidney. Testosterone raises it through several routes, most of which are not kidney injury:

  1. Increased muscle mass increases production: This is the dominant mechanism. Bhasin (2001, PMID 11701431) demonstrated that fat-free mass increases in a linear dose-response with testosterone, rising 3.4, 5.2 and 7.9 kg at 125, 300 and 600 mg per week respectively. More muscle produces more creatinine at the same kidney function.

  2. Creatine supplementation compounds it: Many men on testosterone also supplement creatine, which raises serum creatinine directly without affecting kidney function. This is an extremely common confounder.

  3. Modest RAAS activation: Androgens have some effect on the renin-angiotensin-aldosterone system and on renal haemodynamics, which can raise creatinine slightly independently of muscle mass.

  4. eGFR is calculated from creatinine, so it falls automatically: Every eGFR equation in routine use takes creatinine as its main input. When a muscular man's creatinine rises through muscle mass alone, his reported eGFR falls, and the report may label him as having chronic kidney disease when his kidneys are working normally. This is a calculation artefact, not a finding.

  5. The genuine concern that this artefact can conceal: Anabolic steroid use is associated with real renal pathology, including focal segmental glomerulosclerosis in heavy long-term users. A creatinine rise that is dismissed as muscle mass can mask early injury, and the only way to separate the two is to measure something that does not depend on muscle.

Cypionate versus enanthate: no difference.

Expected Changes

Replacement doses (100-200 mg/week):

  • Creatinine typically rises 5 to 15% above baseline over 3 to 12 months
  • A man moving from 90 to 100 micromol/L is a normal finding
  • Reported eGFR falls correspondingly, often from 95 to the low 80s

Supraphysiological doses:

  • Larger rises, tracking the larger increase in lean mass
  • Creatinine of 120 to 140 micromol/L is common in heavily muscled men with entirely normal kidney function

With creatine supplementation:

  • Add a further rise on top, typically 10 to 20 micromol/L, appearing within weeks of starting

What is not normal:

  • A creatinine rise that continues climbing beyond 12 months without a corresponding increase in lean mass
  • A rise accompanied by proteinuria, hypertension, oedema or a falling cystatin C-based eGFR
  • Any rise of more than 30% from baseline

Timing: Follows the muscle mass trajectory, so it rises over the first 6 to 12 months and then plateaus.

Monitoring Guidance

Baseline creatinine and eGFR before starting, ideally alongside a cystatin C if you intend to build substantial muscle. Without a baseline you cannot distinguish a rise from a naturally high starting point.

On protocol:

  • Creatinine every 6 to 12 months as part of a standard panel.

When creatinine is up, order these three things before worrying:

  1. Cystatin C: produced by all nucleated cells at a constant rate and independent of muscle mass. A cystatin C-based eGFR that is normal alongside a creatinine-based eGFR that is low is strong evidence the creatinine is muscle artefact.
  2. Urine albumin to creatinine ratio (ACR): detects glomerular protein leak, which is the early signal of the kind of renal injury associated with heavy anabolic steroid use. A normal creatinine with an abnormal ACR is the combination that matters, and it is missed if only creatinine is checked.
  3. Blood pressure: hypertension both causes and results from renal injury, and it is far more common in this population than glomerular disease is.

Stop creatine for two weeks before testing if you want an uncontaminated creatinine.

Hydration: Test normally hydrated. Dehydration raises creatinine acutely.

Management Strategies

Do not panic about a 5 to 15% rise. In a man who has gained lean mass on testosterone, this is the expected finding and does not represent kidney injury.

Do not accept it uncritically either. The correct response is to measure something that is not muscle-dependent, not to assume.

Get cystatin C and urine ACR:

  • Together these cost little and resolve the question definitively.
  • A normal cystatin C eGFR plus a normal ACR plus normal blood pressure means the creatinine is artefact.
  • An abnormal ACR means see a doctor, regardless of what the creatinine says.

Protect the kidney through the things that actually damage it:

  • Blood pressure control is the highest-yield intervention. Several anabolic compounds raise blood pressure substantially.
  • Adequate hydration, particularly around hard training.
  • Avoid routine NSAID use, which is common in this population and is genuinely nephrotoxic with chronic use.
  • Be cautious with very high protein intakes in anyone with established renal impairment, though this is not a concern in normal kidneys.

Tell your GP you use testosterone and lift: an eGFR flagged as stage 2 or 3 chronic kidney disease in a muscular man is often an equation artefact, and the context changes the interpretation.

A creatinine rise on testosterone is among the most common and most misinterpreted findings on bodybuilding bloodwork.
