How Orforglipron Affects Resting Heart Rate

Orforglipron raises resting heart rate by about 4 to 5 bpm on average, against 0.5 bpm on placebo, and a 2026 network meta-analysis ranked it the highest in the GLP-1 class at up to about 9 bpm on the top trial dose. The rise adds to clenbuterol, T3, trenbolone and stimulants.

The Mechanism

GLP-1 receptors sit on the heart's pacemaker cells (the sinoatrial node) and GLP-1 agonists also shift the balance of the autonomic nervous system towards sympathetic tone. Every drug in the class raises resting heart rate a little.

Why orforglipron sits at the top of the class:

  • It is taken daily and its half-life is about 29 to 49 hours, so receptor exposure stays high with little trough. Oral and once-daily agents were linked to larger rises in a 2026 meta-analysis (exploratory finding).
  • The rise is not explained by dehydration or weight loss alone; it appears early and persists while you stay on the drug.

Expected Changes

On the US label (pooled obesity trials): mean rise of 4 to 5 bpm versus 0.5 bpm on placebo. Tachycardia-type side effects in 3% versus 0.9%.

By dose in ATTAIN-1 (label table): +3.6, +3.9 and +4.6 bpm at 5.5, 9 and 17.2mg, versus +0.6 bpm on placebo.

Against the rest of the class: a 2026 network meta-analysis of non-diabetic adults put orforglipron at about +7.3 bpm versus placebo overall and about +9.3 bpm at the top trial dose, compared with about +3.5 bpm for retatrutide, +3.4 for semaglutide and +2 for tirzepatide (Zhang 2026, PMID 41582189). The network estimate has a wide range (4.5 to 13.9 bpm).

Head-to-head in type 2 diabetes: +3.7 and +4.7 bpm on orforglipron versus +1.0 and +1.5 bpm on oral semaglutide (ACHIEVE-3, PMID 41765029).

Timing: appears within the first weeks, persists while on drug, settles over about a week after stopping given the half-life.

Monitoring Guidance

Before starting: log a 7-day average of morning resting heart rate (lying or seated, before caffeine) and home blood pressure.

While on it: compare your 7-day average with your own baseline after each dose step, not with a population range.

Stacks: clenbuterol, T3, trenbolone, stimulant pre-workouts, high caffeine and retatrutide all raise heart rate. Log them on the same timeline so you know what moved it.

Cardio zones: a 5 to 10 bpm resting shift also shifts your zone 2 reading. Use the talk test, see why your zone 2 heart rate is wrong on clen, T3 and tren.

Management Strategies

  • Rise of 10 bpm or more over your baseline: drop back a dose step, or remove a stimulant from the stack before blaming orforglipron.
  • Resting heart rate persistently above 100, palpitations, chest pain or fainting: stop and see a doctor; ask for an ECG.
  • On clenbuterol or T3 as well: run them in sequence rather than together while you are titrating orforglipron.
  • Check potassium and magnesium if vomiting or diarrhoea has been heavy, because low electrolytes make a fast heart rate more dangerous.
  • Do not use a beta blocker to hide the number without medical advice.

Clinical Significance

A 4 to 9 bpm rise is small for most people but is the largest in the GLP-1 class, and it stacks with every chronotropic drug lifters use in a cut.
