How Nandrolone Phenylpropionate Affects LDL Cholesterol
NPP is the same hormone as deca, and nandrolone is one of the few anabolic steroids with trial evidence showing no significant effect on LDL. High-dose administration left total cholesterol, LDL cholesterol and LDL particle size unchanged. LDL is not where nandrolone does its damage.
The Mechanism
Nandrolone phenylpropionate delivers identical nandrolone to nandrolone decanoate, so the LDL mechanism is the same:

Injectable, no first-pass hepatic loading: The ester is cleaved in tissue and nandrolone reaches the liver at systemic concentrations. Thompson (1989, PMID 2915439) established that route determines whether an androgen raises LDL: oral stanozolol raised it 29% while intramuscular testosterone in the same men lowered it 16%.

Not 17-alpha-alkylated: Nandrolone phenylpropionate carries a 17-beta ester rather than a 17-alpha-alkyl group, so it does not resist hepatic metabolism and does not produce the sustained hepatocyte androgen signal that downregulates LDL receptor expression.

Weak aromatisation, mitigated by the testosterone base: Nandrolone provides little oestrogenic support for hepatic LDL receptors on its own. Because it is almost always run alongside testosterone, the testosterone supplies that oestradiol. An aggressive aromatase inhibitor removes it, and that is the realistic route to an LDL rise on an NPP cycle.

Lipoprotein(a) falls: Both Sattler (2002, PMID 12388173) and Hartgens (2004, PMID 15155420) documented Lp(a) reduction on nandrolone, a favourable and unusual effect on a marker that resists diet and most drugs.
Where nandrolone actually does its damage: HDL, prolactin, and progesterone receptor-mediated sexual function. Not LDL.
What the ester changes: half-life of roughly 2 to 3 days versus 6 to 12 for decanoate, so steady state within a week and clearance within days.
Expected Changes
High-dose nandrolone for 12 weeks (Sattler 2002, PMID 12388173, randomised, 30 men):
- Total cholesterol: no significant change
- LDL cholesterol: no significant change
- LDL phenotype (particle size distribution): no significant change in the nandrolone-alone group; the group that also did resistance training gained 5.2 angstroms of LDL particle size, a favourable shift
- Triglycerides fell by 66 mg/dL across the whole population
- HDL fell 8.7 to 10.6 mg/dL
At 200 mg/week for 8 weeks (Hartgens 2004, PMID 15155420, randomised double-blind, placebo-controlled):
- No significant change in total cholesterol, triglycerides, HDL, HDL2, HDL3, apoA-I or apoB
- Lp(a) fell from 103 to 65 U/L
- This is the cleanest trial of nandrolone alone in trained athletes and it found essentially nothing on the lipid panel
In real-world stacks: If LDL is up on an NPP cycle, look at what is stacked alongside it. Nandrolone is the least likely compound on the panel to be responsible.
Timing with the short ester: steady state within a week; there is no need to test early specifically for LDL, since two trials suggest there is nothing to find.
Monitoring Guidance
Baseline lipid panel before the cycle, so later changes can be attributed correctly.
On cycle:
- Check at week 4 to 6. There is no need for early or frequent LDL testing on nandrolone specifically.
- Direct monitoring effort at HDL, prolactin and haematocrit, which are the markers nandrolone genuinely moves.
If LDL rises on an NPP cycle, investigate in this order:
- Oral 17-alpha-alkylated compounds in the stack. Stanozolol raised LDL 29% at just 6 mg/day.
- Aromatase inhibitor dosing. Suppressing oestradiol removes hepatic LDL receptor support, and nandrolone contributes little oestradiol of its own to spare.
- Diet and body composition changes over the same period.
Consider Lp(a) once: largely genetically fixed, diet-resistant, and lowered by nandrolone. Worth knowing your baseline number.
Post-cycle: Recheck at 6 to 8 weeks. The short ester means recovery begins within days of the last injection.
Management Strategies
Do not treat nandrolone as the lipid problem: Two randomised trials found no significant LDL effect. If lipid harm reduction is the goal, dropping the oral from the stack achieves far more.
Manage oestradiol properly: Nandrolone aromatises weakly, so the oestradiol on an NPP and testosterone cycle comes from the testosterone. Suppressing it aggressively removes hepatic LDL receptor support without much offsetting benefit.
Standard measures:
- Soluble fibre 10 to 25 g/day lowers LDL through bile acid sequestration
- Replacing saturated fat with unsaturated fat works normally
- Cardiovascular exercise 30 to 45 minutes, 4 to 5 times per week, which in Sattler's trial was associated with a favourable increase in LDL particle size
Direct monitoring where the risk actually is: prolactin, haematocrit, HDL and sexual function are the endpoints with real signal on nandrolone. LDL is not one of them.
Ester advantage: NPP clears within days, so if a lipid or prolactin result requires stopping the compound, it responds quickly. Decanoate does not.

Clinical Significance
Nandrolone is one of very few anabolic steroids for which the LDL question has been answered directly, and the answer is that it does not move. Sattler (2002, PMID 12388173) gave high-dose nandrolone for 12 weeks and found no significant change in total cholesterol, LDL cholesterol or LDL phenotype, while HDL fell and lipoprotein(a) fell. Hartgens (2004, PMID 15155420) gave 200 mg per week in a placebo-controlled design and found no significant change in any lipid or apolipoprotein measured, including apoB. Nandrolone phenylpropionate is the same hormone on a shorter ester and behaves identically. The clinical value of this is attributional: a rising LDL on an NPP-containing cycle almost always reflects an oral compound or an over-suppressed oestradiol, and correcting either is far more productive than reducing the nandrolone. The compound's real monitoring burden sits with prolactin, haematocrit and HDL.
