Threshold: above 54% is the conventional point for intervention, because blood viscosity rises steeply and with it the risk of stroke, deep vein thrombosis, pulmonary embolism and myocardial infarction.

Monitoring Guidance

Baseline full blood count before the cycle. Men starting above 50% are at high risk of crossing 54%.

On cycle:

  • Full blood count at week 4 and again at week 8 to 10.
  • Sample at trough and normally hydrated. Dehydration falsely elevates haematocrit.

  • Therapeutic phlebotomy or blood donation removes one unit and typically lowers haematocrit by 3 to 4 points
  • Reduce the overall androgen load
  • With the short ester, stopping NPP produces a measurable improvement within 2 to 3 weeks, which is a real advantage over decanoate

Do not treat donation as free: repeated phlebotomy depletes ferritin. Track it, and reduce the need for donation through dose management rather than relying on it.

Ester choice: If you have a haematocrit history, NPP is the more manageable of the two nandrolone esters, because the number responds when you act. That is worth more than the convenience of less frequent injections.
