How Imeglimin Affects Fasting Glucose

Imeglimin lowers glucose mainly by boosting insulin release when glucose is high, so it has a low risk of hypoglycaemia on its own. All data comes from people with type 2 diabetes.

The Mechanism

Imeglimin's main human effect is on the pancreas. In a 7-day clamp study it raised the glucose-stimulated insulin response by 112% and beta-cell glucose sensitivity by 36% (Pacini et al., 2015). In rodent islets this involved a larger NAD+ pool via the NAMPT salvage pathway. Because the effect depends on glucose being high, insulin output falls back as glucose normalises.

A smaller, uncontrolled study of 22 men with type 2 diabetes also found improved insulin sensitivity in muscle, liver and fat after 20 weeks, and a lower glucose curve on an oral glucose tolerance test, visible within one week (Tajima et al., 2026).

Expected Changes

  • Diabetic patients: fasting glucose fell from 148.5 to 131.0 mg/dL (8.2 to 7.3 mmol/L) over six months in a 28-patient pilot (Fujita et al., 2026)
  • Glucose tolerance: the 3-hour OGTT glucose area fell by a median 108.6 mg·h/dL at 20 weeks, with most of the change already present at week 1
  • Hypoglycaemia: similar to placebo even when added to insulin (TIMES 3)
  • Non-diabetic lifters: no data. From a normal fasting glucose, expect little change

Monitoring Guidance

  • Baseline: fasting glucose, fasting insulin and HOMA-IR, drawn 8-12 hours fasted, no GH injection in the previous 5 hours
  • Week 1-2: optional fasting glucose check, since the effect appears quickly
  • Week 4-6 and 12: fasting glucose, insulin, HOMA-IR and fructosamine
  • Kidney function: imeglimin is cleared unchanged by the kidneys; check cystatin C at baseline, as creatinine-based eGFR is unreliable in muscular people

Management Strategies

  • If glucose is elevated because of MK-677 or GH, reduce the cause first; imeglimin is not a licence to keep the dose high
  • Fasting glucose of 7.0 mmol/L or higher on two tests is diabetic range: see a doctor
  • Combining with metformin raises GI side effects to 38-41% in Japanese studies
  • Combining with insulin or sulfonylureas raises hypoglycaemia risk; do not self-experiment with that combination

Clinical Significance

Imeglimin lowers glucose in type 2 diabetes with a low hypoglycaemia risk on its own. Its effect on normal glucose in healthy lifters has never been studied.
