How Gestrinone Affects ALT

Raised liver enzymes appeared in 15.1% of reports in a 2025 gestrinone safety review, but ALT and AST stayed normal in a 24-woman high-dose study. Check a baseline and a 6-week ALT, especially with an implant.

The Mechanism

Gestrinone is not a 17-alpha-alkylated oral like oxandrolone or stanozolol, so it does not carry the classic oral-steroid liver load. Like other androgens and progestins, it is metabolised by the liver, and some users show mild transaminase rises.

With compounded implants, the dose is unknown and pellets were often combined with oxandrolone, which does raise ALT. Any rise on a combined implant cannot be blamed on gestrinone alone.

Expected Changes

  • Safety review (Fagundes 2025): raised transaminases in 15.1% of pooled reports. No fold-change data
  • High-dose fibroid study (Coutinho 1990, n=24): ALT and AST stayed within the normal range
  • GLADE implant trial: uses ALT or AST above 3 times the upper limit of normal at 3 months as a safety outcome. Results not yet published

Most users should see no change or a mild rise. A large rise points to another cause or another compound.

Monitoring Guidance

  • ALT and AST at baseline
  • Repeat at 6-8 weeks, then every 3 months
  • Avoid hard training for 48-72 hours before the draw: muscle damage raises AST and, to a lesser degree, ALT
  • Add GGT if ALT rises, to separate liver from muscle

Management Strategies

  • ALT up to 2 times the upper limit: recheck in 4 weeks, cut alcohol, review other drugs and supplements
  • ALT above 3 times the upper limit: stop the drug or remove the implant, and see a doctor for a liver workup
  • Ask whether your implant also contained oxandrolone, which is a known cause of ALT rises

Clinical Significance

Liver enzyme rises on gestrinone are usually mild and uncommon. A rise above 3 times the upper limit is the threshold used in the GLADE implant trial.
