Effects on Markers

Produces brief testosterone spikes with a return toward baseline between doses, aromatises to estradiol, largely preserves LH/FSH and sperm production, minimal effect on haemoglobin/haematocrit

Compound Guide

Route: Intranasal. A 4.5% gel delivered by a metered applicator into each nostril. Absorbed through the nasal mucosa straight into circulation.

Dosage:

  • Natesto: 5.5 mg per nostril, 11 mg per administration, three times daily = 33 mg/day
  • Some protocols drop to twice daily on retest, but the label schedule is three times daily
  • Doses should be spaced 6-8 hours apart

Application Protocol:

  • Blow your nose first, then prime the pump per the device instructions
  • Insert the applicator, depress fully, and apply to the inner nostril wall (the septum side)
  • Do not sniff or blow your nose for at least an hour afterwards
  • Wipe the applicator tip after each use

The Fertility Angle (why this compound exists):

  • The short pulsatile profile does not sustain the constant negative feedback that flattens LH and FSH on injectables
  • Studies show preserved gonadotropins, preserved intratesticular testosterone, and preserved sperm parameters in most users

Bloodwork Timing:

  • Draw roughly 45-60 minutes after a dose for a peak, and record the interval
  • Judge the protocol on symptoms plus LH, FSH, and a semen analysis if fertility is the goal, not on a single total testosterone number

Key Notes:

  • Nasal side effects are the limiter: rhinorrhoea, nasal discomfort, epistaxis, and scabbing inside the nostril
  • Not for anyone with significant sinus disease, nasal polyps, or a history of nasal or sinus surgery
  • Three doses a day is a real adherence burden and the usual reason people stop
  • No transfer risk to partners or children
