Sustanon 250 vs Testosterone Enanthate: Is the Blend Worth It?
Sustanon 250 is a four-ester testosterone blend; testosterone enanthate is a single ester. At the same weekly milligrams they deliver almost the same amount of testosterone, so the real differences are how fast it kicks in, how much levels swing between injections, and how long it takes to clear.
Overview
Sustanon 250 and testosterone enanthate (Test E) are both oil-based intramuscular testosterone. The hormone they release is identical. What differs is the ester, which controls how quickly testosterone leaves the injection depot.
Sustanon 250 (Aspen, formerly Organon) contains four esters per 1 mL, in arachis (peanut) oil with benzyl alcohol:
- 30 mg testosterone propionate
- 60 mg testosterone phenylpropionate
- 60 mg testosterone isocaproate
- 100 mg testosterone decanoate
Testosterone enanthate is a single ester, usually 250 mg/mL outside the US or 200 mg/mL in the US, typically in sesame or castor oil.
Sustanon was designed for one injection every three weeks. In practice, most TRT clinics and most lifters inject it weekly or twice weekly, at which point it behaves very much like enanthate.
Side-by-Side Comparison
| Attribute | Sustanon 250 | Testosterone Enanthate |
|---|---|---|
| Esters | Propionate, phenylpropionate, isocaproate, decanoate | Enanthate only |
| Concentration | 250 mg/mL | 200 or 250 mg/mL |
| Testosterone per 250 mg | 176 mg | About 180 mg |
| Single-dose peak | 24 to 48 hours (about 70 nmol/L) | Within about 2 days (reported) |
| Apparent half-life | No single value (blend) | About 4.2 days |
| Labelled schedule | Every 3 weeks | Every 2 to 4 weeks (US label) |
| Typical real-world schedule | Weekly or twice weekly | Weekly or twice weekly |
| Carrier oil | Arachis (peanut) oil | Sesame or castor oil |
| Clearance after stopping | Slower (decanoate tail) | Faster |
Key Differences
Testosterone content:
- Sustanon 250: 176 mg of actual testosterone per 250 mg (70.5%), per the UK SmPC
- Enanthate: about 72 mg per 100 mg, so 180 mg per 250 mg
- At 500 mg a week that is about 352 mg vs 360 mg of testosterone, a 2% difference
Release profile:
- Sustanon's propionate peaks within a day or two; a single dose peaks at about 70 nmol/L at 24 to 48 hours and falls to the low end of normal in about 21 days (SmPC)
- Enanthate has an apparent half-life of about 4.2 days and typically peaks within the first two days after injection
- Sustanon's decanoate tail means it takes longer to reach steady state and longer to clear when you stop
Peaks and troughs:
- On a weekly schedule the propionate component likely produces a sharper early peak and a deeper dip before the next shot than enanthate alone
- Splitting the dose twice weekly flattens both
Carrier oil:
- Sustanon uses arachis oil, so it is unsuitable for anyone with a peanut or soya allergy (the SmPC contraindicates it)
- Enanthate is usually in sesame or castor oil
Injection comfort:
- Users widely report that propionate stings more, so some find Sustanon slightly more painful; there is no study measuring this
- UGL "Sustanon 400" or "Test 400" products are a separate issue: at 400 mg/mL they carry more co-solvent and are widely reported to cause much more post-injection pain
When to Use Which
Choose Sustanon 250 if:
- It is what your prescriber or pharmacy supplies (it is a standard prescription product in the UK and several other countries)
- You want a slightly faster onset at the start of a cycle, thanks to the short esters
- You inject at least weekly; the three-weekly label schedule gives large swings
Choose testosterone enanthate if:
- You want the simplest, most predictable bloodwork: one ester, one half-life
- You have a peanut or soya allergy
- You may want to stop or adjust quickly (no decanoate tail)
- You are on TRT and want stable levels with twice-weekly or more frequent injections
Switching between them: keep the same weekly milligrams. Test at trough 6 to 8 weeks after switching, because Sustanon's decanoate takes longer to settle than enanthate.
Clinical Context
Both are approved testosterone replacement products in many countries. The Sustanon 250 SmPC describes a single dose peaking at about 70 nmol/L within 24 to 48 hours and returning to the lower limit of normal in about 21 days, and contraindicates it in peanut or soya allergy. For stable TRT levels, clinicians commonly give Sustanon weekly or every two weeks rather than every three. Monitoring is the same for both: trough total testosterone, haematocrit and estradiol, starting 6 to 8 weeks after any change.
Bodybuilder Context
On a cycle, Sustanon and enanthate are used at the same weekly doses (commonly 500 to 750 mg) and deliver almost the same amount of testosterone. Sustanon's reputation for a faster kick comes from its propionate and phenylpropionate; its reputation for being harder to time bloodwork around comes from the decanoate. If you want clean, interpretable labs, enanthate is easier. If Sustanon is what you can get from a pharmacy rather than an underground lab, that pharmaceutical quality is worth more than any ester difference.
