Imeglimin vs Metformin: same HbA1c, different mechanism, very different evidence base
Imeglimin (Japan, 2021) lowers HbA1c about as much as metformin, mainly by boosting glucose-dependent insulin secretion, with a milder mitochondrial effect. Metformin has decades of human safety data; imeglimin has small diabetic trials and no data in lifters.
Overview
Metformin is the default insulin sensitiser in bodybuilding, used mostly to offset insulin resistance from GH, MK-677 or heavy carbohydrate intake. Imeglimin is the first drug in a newer class, the glimins, approved in Japan as Twymeeg in 2021 and in India from late 2022. It shares a chemical fragment with metformin but is not a biguanide.
The two get compared because both act on mitochondria and both lower HbA1c. Imeglimin is marketed in some circles as "metformin plus MOTS-c plus NAD+", with metformin's gut, lactate and training problems solved. The trial data supports part of that, and the rest comes from animals.
Every imeglimin trial was done in people with type 2 diabetes, mostly in Japan. There is no study in healthy or trained people.
Side-by-Side Comparison
| Attribute | Imeglimin | Metformin |
|---|---|---|
| Drug class | Glimin (tetrahydrotriazine) | Biguanide |
| Main action | Glucose-dependent insulin secretion; mild mitochondrial modulation | Lowers liver glucose output; Complex I inhibition |
| HbA1c reduction | 0.4-0.9% in diabetic trials | About 1-1.5% in diabetic trials |
| Hypoglycaemia alone | Low | Low |
| GI side effects | Low alone; 38-41% combined with metformin | Common early, reduced by XR |
| Lactic acidosis risk | Lower in animal models only | Rare but established |
| Training interference | No human data; protective in mouse atrophy models | Blunted hypertrophy in adults over 65 |
| Lab quirk | HbA1c may read high early (longer red cell lifespan) | Vitamin B12 depletion |
| Half-life | 9-20 h (Western), 4.5-12 h (Japanese) | 5-6 h (IR), 8-12 h (XR) |
| Approval | Japan, India | Worldwide |
Key Differences
Primary mechanism: Metformin mainly lowers hepatic glucose output and is a strong mitochondrial Complex I inhibitor. Imeglimin mainly amplifies insulin secretion when glucose is high (+112% glucose-stimulated insulin response in a 7-day clamp study, Pacini et al., 2015) and only partially inhibits Complex I. In intact rat liver cells, metformin cut oxygen consumption and imeglimin did not (Vial et al., 2021).
Glucose lowering: Similar. Imeglimin lowered HbA1c by 0.87% vs placebo as monotherapy (TIMES 1) and 0.44% vs placebo when added to metformin. In head-to-head work it matched 1000 mg/day metformin (Usui et al., 2025) and beat a metformin dose increase by 0.21% (MEGMI, Takahashi et al., 2025).
Insulin sensitivity: Metformin's is well established. Imeglimin improved muscle, liver and fat insulin sensitivity on clamp in one 22-man, single-arm, sponsor-funded study (Tajima et al., 2026).
GI tolerance: Imeglimin alone causes fewer GI problems than metformin. Combined, GI side effects reached 38-41% in Japanese studies, and 7 of 33 people stopped imeglimin in MEGMI.
Lactic acidosis: Only metformin raised lactate and caused deaths in dog and rat models (Theurey et al., 2022, developer-funded). No human comparison exists.
Muscle and training: Metformin at 1700 mg/day blunted lean mass gains from resistance training in adults over 65 (MASTERS). Imeglimin has no human training data; in mice it protected against muscle atrophy in obesity, ageing and fatty liver disease.
Kidney clearance: Both are cleared by the kidneys. Imeglimin is excreted unchanged and its dose is halved at eGFR 15-45.
Lab quirks: Metformin depletes vitamin B12 over time. Imeglimin appears to lengthen red cell lifespan, making HbA1c read high for about two months after starting (INFINITY trial).
Regulatory status and cost: Metformin is approved everywhere and costs almost nothing. Imeglimin is approved only in Japan and India and is a personal import elsewhere.
When to Use Which
Metformin makes more sense if:
- You tolerate it and your fasting glucose, insulin and HOMA-IR respond
- You want the drug with decades of human safety data
- You need a cheap, legal option a GP will prescribe
Imeglimin is worth discussing with a prescriber if:
- Metformin genuinely wrecks your gut even in XR form
- You understand that every efficacy number comes from diabetic patients
- You can source it legitimately and will monitor fructosamine, cystatin C and liver enzymes
Do not:
- Stack the two without medical supervision, since GI side effects reached 38-41% in Japanese studies
- Use imeglimin to "fix" HOMA-IR from MK-677 or GH before reducing the cause
- Judge imeglimin by HbA1c in the first two months
Clinical Context
Clinically, imeglimin is a second-line oral agent in Japan, often added to other drugs including DPP-4 inhibitors and insulin, where its low hypoglycaemia risk is useful. Metformin remains first-line almost everywhere because of cost, long-term safety and outcome data. The two are not interchangeable mechanistically: metformin works mostly on the liver, imeglimin mostly on the beta cell, which is why imeglimin can add HbA1c reduction on top of metformin (-0.44% in a phase 2 add-on trial). Imeglimin needs dose reduction in moderate to severe kidney impairment and is not recommended below an eGFR of 15 without adjustment.
Bodybuilder Context
For lifters, the question is usually whether imeglimin can replace metformin for GH- or MK-677-driven insulin resistance without metformin's gut problems or training interference. Honest answer: possibly, but nobody has tested it in that population. The metformin training penalty comes from older adults on 1700 mg/day; most lifters use 500 mg with dinner, where the effect in young trained people is unknown. If you do run imeglimin, use fructosamine rather than HbA1c for the first recheck, get cystatin C rather than relying on creatinine-based eGFR, and watch ALT and AST.
