Anti-Cyclic Citrullinated Peptide Antibody
An antibody directed against citrullinated proteins that is highly specific for rheumatoid arthritis and often appears years before symptoms. Positivity supports the diagnosis and predicts a more erosive disease course.

Anti-CCP is not affected by AAS, GH, or peptides, but it is worth understanding for lifters. Enhanced athletes frequently have aches, joint pain, and stiffness from heavy training, prior injuries, and osteoarthritis, and it is easy to dismiss genuine inflammatory arthritis as normal gym wear and tear. A positive anti-CCP points to rheumatoid arthritis rather than mechanical joint pain and warrants rheumatology review. Units and cutoffs are strongly assay-dependent, so always read the result against the reporting laboratory's own reference interval. The bodybuilder range equals the standard range.
When high
When positive/elevated (above the assay cutoff):
- A positive anti-CCP is highly specific for rheumatoid arthritis and can precede joint symptoms by years. It also predicts more aggressive, erosive disease.
- What to do:
- Seek rheumatology assessment, especially if there is symmetrical small-joint pain and swelling, prolonged morning stiffness (>30-60 minutes), or fatigue.
- Interpret alongside rheumatoid factor, CRP, and ESR; the combination of anti-CCP plus rheumatoid factor is more specific than either alone.
- Do not attribute persistent inflammatory joint symptoms to training alone if anti-CCP is positive.
- Early diagnosis and disease-modifying treatment protect joints, so do not delay specialist review.
