How Oxandrolone Affects LDL Cholesterol
Oxandrolone raises LDL, and unlike most anabolic steroids there is a randomised trial that measured it directly: 20 mg/day for 12 weeks increased directly measured LDL by 0.57 mmol/L (roughly 22 mg/dL) and non-HDL cholesterol by 0.54 mmol/L. The 'mild oral' reputation does not extend to LDL.
The Mechanism
Oxandrolone raises LDL through the same route-dependent hepatic mechanism as other 17-alpha-alkylated orals, at a somewhat lower intensity per milligram:

First-pass hepatic exposure: Oxandrolone is 17-alpha-alkylated so that it survives the liver intact. The consequence is a repeated concentrated androgen signal at the hepatocyte, which reduces LDL receptor expression and slows LDL clearance.

No aromatisation: Oxandrolone is a dihydrotestosterone derivative and does not convert to oestradiol. Oestradiol upregulates hepatic LDL receptors, so a non-aromatising compound offers no counterbalance to the androgenic downregulation.

Concurrent HDL suppression via hepatic lipase: The same hepatic androgen signal upregulates hepatic triglyceride lipase, which catabolises HDL. This is why oxandrolone moves LDL up and HDL down at the same time, worsening non-HDL cholesterol on both sides of the equation.

Milder per milligram, not milder in effect: Oxandrolone's reputation for mildness reflects lower androgenic potency per milligram than stanozolol or oxymetholone. Bodybuilding doses of 40 to 80 mg/day are three to four times the 20 mg/day that produced a 0.57 mmol/L LDL rise in a controlled setting, which erases most of that margin.
Expected Changes
At 20 mg/day (Schroeder 2004, PMID 15472177, 12 weeks, randomised, 32 men aged 60-87):
- Directly measured LDL cholesterol: +0.57 mmol/L (approximately +22 mg/dL), p less than 0.03
- Non-HDL cholesterol: +0.54 mmol/L (approximately +21 mg/dL)
- HDL cholesterol: -0.49 mmol/L (approximately -19 mg/dL)
- The authors noted these changes were largely reversible after stopping.
- This is a directly measured LDL, not a Friedewald calculation, which matters because calculated LDL is unreliable when HDL and triglycerides are both shifting.
At 10 mg/day and below:
- Davis (2017, PMID 27802097) gave prepubertal boys with Klinefelter syndrome 0.06 mg/kg/day for two years. HDL fell significantly and triglycerides fell; the trial was in children at a very low dose and does not translate to adult bodybuilding use, but it confirms the lipid signal is present even at replacement-scale dosing.
At 40-80 mg/day (typical bodybuilding use):
- No trial has measured LDL at these doses. Extrapolating the dose-response from 20 mg/day, an LDL rise of 1 mmol/L (roughly 39 mg/dL) or more is plausible, alongside HDL falling into the teens.
- Glazer's (1991, PMID 1929679) pooled weighted average across anabolic steroids was a 36% LDL rise, which is the sensible class-level expectation.
Timing: LDL climbs over the first 3 to 4 weeks and plateaus. Recovery is relatively quick after cessation, generally 8 to 12 weeks.
Monitoring Guidance
Baseline: Lipid panel before starting, including HDL and triglycerides so that any calculated LDL can be interpreted properly.
Ask for a direct LDL if you can: Schroeder's trial used directly measured LDL for good reason. When HDL is falling and triglycerides are moving, the Friedewald equation used by most labs to calculate LDL becomes less reliable. If your lab offers direct LDL or ApoB, use it.
On cycle:
- Check at week 4 of a 6 to 8 week cycle. Both LDL and HDL have moved by then.
- Track non-HDL cholesterol (total minus HDL). It captures both halves of the change in one number and requires no extra test.
Women on oxandrolone:
- Oxandrolone is the compound most commonly used by women, often at 5 to 20 mg/day. The lipid effect is not sex-sparing and the pre-cycle HDL is typically higher, so the absolute fall can be larger. Do not skip the lipid panel because the dose sounds small.
Post-cycle: Recheck at 8 to 12 weeks. Schroeder reported the changes were largely reversible.
Management Strategies
Dose is the main lever:
- The measured 22 mg/dL LDL rise came from 20 mg/day. Most bodybuilding protocols use two to four times that. Halving the dose is the single most reliable way to halve the lipid cost.
- Keep cycles to 6 to 8 weeks.
Do not stack a second oral: The hepatic load and the LDL receptor downregulation are additive, and the liver enzyme picture becomes hard to interpret at the same time.
Supportive measures with actual evidence:
- Cardiovascular exercise 30 to 45 minutes, 4 to 5 times per week.
- Soluble fibre 10 to 25 g/day works through bile acid sequestration, a pathway oxandrolone does not touch.
- Replacing saturated fat with unsaturated fat lowers LDL independently of anything the compound is doing.
- Omega-3 at 3 to 4 g/day is for triglycerides, not LDL. It will not fix this number.
What does not work:
- Believing oxandrolone is lipid-neutral because it is described as mild. Users who report no cholesterol effect on anavar are usually reporting that they did not test.

Clinical Significance
Oxandrolone has better LDL evidence than almost any other anabolic steroid because Schroeder (2004, PMID 15472177) used directly measured LDL rather than a calculation, in a randomised design, at a defined dose. A 0.57 mmol/L rise in LDL alongside a 0.49 mmol/L fall in HDL at only 20 mg/day means non-HDL cholesterol moved by more than half a millimole in 12 weeks. That is a clinically meaningful shift in atherogenic burden from a compound routinely described as mild, and typical bodybuilding doses are three to four times higher than the dose tested. The reversibility reported in the trial is genuine and is the main mitigating factor, but it applies to the lipid numbers, not to cumulative arterial exposure across repeated cycles.
