How Methandrostenolone Affects LDL Cholesterol

Bodybuilders using 50-100 mg/day of methandrostenolone in a stack had LDL of 150 mg/dL on cycle versus 125 mg/dL off, with an LDL to HDL ratio of 6.0 versus 2.2. Dianabol aromatises, which partly protects LDL, but it is still a 17-alpha-alkylated oral and the ratio damage is severe.

The Mechanism

Methandrostenolone (dianabol) raises LDL through the standard 17-alpha-alkylated oral pathway, with one partial mitigating feature that the other orals lack:

  1. First-pass hepatic androgen loading: The 17-alpha-alkyl group lets the compound survive first-pass metabolism, which means hepatocytes receive a concentrated androgen signal after each dose. This downregulates hepatic LDL receptor expression and slows LDL particle clearance.

  2. It aromatises, which helps: Unlike stanozolol, oxandrolone or oxymetholone, methandrostenolone converts to methylestradiol, an oestrogenic metabolite. Oestradiol upregulates hepatic LDL receptors. Zmuda (1993, PMID 8487666) showed that removing aromatisation from a testosterone protocol worsened lipids across the board, and the same logic works in reverse here: dianabol's oestrogenicity partly offsets its own hepatic signal. This is the mechanistic reason dianabol's lipid profile, while bad, is not as extreme as stanozolol's.

  3. Simultaneous HTGL upregulation crushes HDL: The hepatic androgen signal drives hepatic triglyceride lipase, which catabolises HDL particles. Because LDL rises while HDL falls, the ratio deteriorates roughly twice as fast as either marker alone.

  4. Aromatase inhibitors remove the protection: Users commonly run an aromatase inhibitor alongside dianabol to control bloating and gynecomastia risk. Doing so removes the one lipid-protective feature the compound has. This is a genuinely underappreciated trade-off: controlling oestrogenic side effects makes the lipid picture worse.

Expected Changes

Bodybuilders on 50-100 mg/day dianabol in a stack (Webb 1984, PMID 6493049, 14 men, on-drug versus off-drug comparison):

  • LDL cholesterol: 150 plus or minus 44 mg/dL on cycle versus 125 plus or minus 38 mg/dL off (p less than 0.05), roughly a 20% increase
  • HDL cholesterol: 29 plus or minus 8 mg/dL on cycle versus 61 plus or minus 14 mg/dL off, a fall of more than half
  • LDL to HDL ratio: 6.0 plus or minus 3.7 on cycle versus 2.2 plus or minus 1.0 off, nearly triple
  • HDL was at or below the age, race and sex-specific 10th percentile in 11 of 14 men while on drug

Important caveat on that data: the Webb cohort also took 100-200 mg/week testosterone and 100-200 mg/week nandrolone decanoate. The LDL rise cannot be attributed to methandrostenolone alone. Given that injectable testosterone lowered LDL 16% in Thompson (1989, PMID 2915439) and high-dose nandrolone had no significant LDL effect in Sattler (2002, PMID 12388173), the oral is the most plausible driver, but this is inference rather than an isolated measurement.

Class-level expectation:

  • Glazer (1991, PMID 1929679) pooled the anabolic steroid lipid literature and reported a weighted average LDL rise of 36%.

Typical dianabol-only doses (20-50 mg/day):

  • Expect LDL to rise 15 to 30% and HDL to fall 40 to 60% over a 4 to 6 week run.
  • The LDL to HDL ratio is the number that moves most dramatically and is the one to watch.

Timing: Measurable by week 2, plateau by week 4. Recovery over 6 to 12 weeks after stopping.

Monitoring Guidance

Baseline: Lipid panel before starting. Dianabol runs are short, so without a baseline the on-cycle panel is uninterpretable.

On cycle:

  • Check at week 3 to 4, which for a typical 4 to 6 week dianabol run is close to the end.
  • Calculate the LDL to HDL ratio yourself. It is the marker that showed the largest change in the Webb data and the one most labs do not report.
  • Non-HDL cholesterol (total minus HDL) is the other free number worth tracking.

Watch the aromatase inhibitor interaction:

  • If you are running an aromatase inhibitor with dianabol, expect a worse lipid panel than dianabol alone would produce. Recheck lipids specifically after starting or increasing an AI.
  • Do not respond to a bad lipid panel by increasing the AI. That is the wrong direction.

Post-cycle: Recheck 6 to 8 weeks after the last dose. Webb's subjects had been off for at least 4 months when their off-drug values were taken, and had recovered to HDL of 61 mg/dL.

Management Strategies

Cycle design:

  • Keep dianabol runs to 4 to 6 weeks. Cumulative exposure is the variable you control.
  • Do not stack a second oral. The 17-alpha-alkylated compounds are additive on both lipids and liver enzymes.
  • Use the minimum aromatase inhibitor dose that controls symptoms. Aromatisation is protecting your LDL, and crushing oestradiol removes that protection.

During the run:

  • Cardiovascular exercise 30 to 45 minutes, 4 to 5 times per week. This is the strongest non-pharmacological lever, and it works on HDL specifically.
  • Soluble fibre 10 to 25 g/day for LDL, via bile acid sequestration.
  • Replace saturated fat with unsaturated fat for the duration.
  • Omega-3 at 3 to 4 g/day addresses triglycerides, not LDL.

Perspective:

  • An LDL to HDL ratio of 6 is the kind of number that prompts a cardiologist to start statin therapy in a general population patient. Webb's bodybuilders reached it in weeks. The fact that it reverses does not make the exposure free.
