How Eloralintide Affects Potassium and Electrolytes

Eloralintide does not act on the kidney or on electrolyte handling. Potassium falls because appetite suppression cuts food intake, and food is where potassium comes from.

The Mechanism

This is an indirect effect and it is worth being precise about that, because a lot of monitoring advice treats every marker change as pharmacology.

Eloralintide has no known action on renal potassium handling, aldosterone, or any electrolyte transporter. What it has is potent appetite suppression, and in the Phase 2 trial decreased appetite was among the most common treatment-emergent effects.

Dietary potassium comes almost entirely from food, concentrated in fruit, vegetables, dairy and meat. Cut total intake by a third and potassium intake falls with it. The same applies to magnesium, and to sodium in anyone who was getting most of theirs from prepared food.

Two things compound it in an athlete. Sweat losses continue at training volume even as intake drops. And anyone using a diuretic, whether pharmaceutical or a "water shedding" supplement, is losing potassium through a second route at the same time.

Expected Changes

Potassium: a gradual drift toward the lower end of range in anyone whose intake has dropped sharply. Frank hypokalaemia is uncommon on food restriction alone but becomes realistic when a diuretic is added.

Magnesium: often the first to move, and the most likely to be low before anyone thinks to check. Serum magnesium also underestimates total body depletion, so a low-normal reading in a heavily restricted athlete deserves attention rather than reassurance.

Sodium: usually unremarkable unless intake has collapsed or fluid intake is very high relative to it.

Timing: this is a weeks-to-months effect that tracks how aggressively intake fell, not a dose effect.

Monitoring Guidance

Baseline electrolyte panel before starting, particularly if you already run a diuretic or compete.

At 4 weeks if appetite suppression has genuinely cut your intake. This is the single highest-yield early check on an appetite suppressant, and the one most often skipped.

At 8 to 12 weeks with the full repeat panel.

Sooner if you develop muscle cramps, unusual fatigue, palpitations, or notice a drop in training performance that does not track your calorie deficit.

Draw at trough alongside the rest of the panel so the timing stays consistent.

Management Strategies

If potassium is low-normal and falling: address intake before reaching for a supplement. Potassium-rich whole foods are more effective and far safer than potassium tablets, which carry a real risk of cardiac events when overdone.

If you are using a diuretic: stop treating the electrolyte panel as optional. This is the combination that produces genuinely dangerous readings, and it is common in the weeks before a show.

If magnesium is low-normal: supplementation is low-risk and reasonable. Glycinate or citrate at 200 to 400 mg elemental daily, taken away from other minerals that compete for absorption.

Do not fix this by eating more salt. Sodium is rarely the problem in this scenario and adding it does nothing for potassium or magnesium.

Clinical Significance

The effect is indirect but the consequences are real. Potassium disturbance is arrhythmogenic, and the risk concentrates in athletes combining an appetite suppressant with a diuretic, which is a common pre-contest pattern. This is the most under-monitored change on any appetite-suppressing compound.
