How Cagrilintide Affects Insulin and Hypoglycaemia Risk
Cagrilintide is a dual amylin and calcitonin receptor agonist. It reduces fasting insulin as weight comes off, but its most clinically important insulin interaction is not the fasting number: amylin suppresses meal-induced glucagon release, which removes the primary defence against falling glucose. In a patient on exogenous insulin, that transfer of pramlintide's precedent boxed warning matters more than any bloodwork change.
The Mechanism
Amylin is co-secreted with insulin from pancreatic beta cells and normally acts as a brake on both post-meal glucose excursions and glucagon release. Cagrilintide is engineered as a long-acting amylin analog with additional agonism at the calcitonin receptor, and it produces two distinct insulin-relevant effects.
Insulin sensitisation via weight loss. In REDEFINE 1 (Garvey et al., 2025, NEJM) cagrilintide monotherapy at 2.4 mg weekly produced 11.5 percent weight loss over 68 weeks. Fasting insulin falls as fat mass drops, and HOMA-IR improves accordingly. This is the standard body-composition-driven insulin effect and it applies to any weight-loss agent.
Glucagon suppression, which is the more important story. Amylin analogs slow meal-induced glucagon release, which is part of how they flatten the post-meal glucose curve. It also means that when glucose does fall, the body's primary counter-regulatory hormone is not fully available. Pramlintide, the only previously approved amylin analog, carries an FDA boxed warning for severe hypoglycaemia when combined with exogenous insulin. In the pivotal Ratner et al. 2004 trial, severe hypoglycaemia hit 16.8 percent of type 1 diabetes patients on pramlintide plus insulin over the first three months, versus 10.8 percent on insulin alone. Events clustered within two to three hours of injection. The mandated mitigation is a 50 percent reduction in mealtime insulin at initiation, then re-titration.
Cagrilintide has not been trialled in insulin users, so the exact magnitude of hypoglycaemia risk is uncalibrated. Mechanistically, the effect transfers.
Expected Changes
Fasting insulin, non-diabetic user. Expect a modest reduction that tracks weight loss over 12 to 24 weeks. A typical cagrilintide 2.4 mg user losing 10 percent of body weight will see fasting insulin fall by roughly 25 to 40 percent, HOMA-IR by a similar fraction. The change is not amylin-specific; any effective weight-loss agent produces it.
Fasting insulin, T2D user. In REDEFINE 2, CagriSema improved HbA1c to below 6.5 percent in 73.5 percent of participants over 68 weeks (Davies et al., 2025, NEJM). Fasting insulin drops proportionally.
Post-meal glucose curve. Cagrilintide flattens the post-meal spike through slowed gastric emptying and amylin's central effects on satiety. Continuous glucose monitor traces show reduced amplitude of post-prandial excursions.
Timing. Insulin reductions and HOMA-IR improvements are typically measurable by week 12 and stable by week 24. Amylin's meal-glucose effects are immediate from the first dose.
In an insulin user, the change that matters is not on a lab report. Severe hypoglycaemia risk clusters within two to three hours of the meal-time insulin injection when meal-induced glucagon fails to fire. This is a symptom-and-CGM problem, not a fasting-panel problem.
Monitoring Guidance
Baseline before starting cagrilintide. Fasting glucose, fasting insulin, HbA1c and HOMA-IR to establish the starting point. If the user is on exogenous insulin, add a continuous glucose monitor for at least two weeks before initiation to characterise the current pattern.
Week 4 check. Fasting glucose and fasting insulin. If the user is not on exogenous insulin, this is straightforward; expect a small directional drop in both.
Week 12 check. Full glucose panel including HbA1c and HOMA-IR. In non-diabetics targeting HOMA-IR below 2.0, expect meaningful improvement by this point.
Week 24 check. Same panel. HbA1c is now reflecting a full quarter of amylin exposure and its improvement is the most reliable single-number signal.
Continuous, if on insulin. Daily CGM tracing in the two to three hour post-meal window is not optional. The pramlintide precedent mandates a 50 percent mealtime insulin reduction at initiation; treat that as the floor, not the ceiling. Do not train fasted while establishing tolerance because a fasted state already lowers counter-regulatory reserve.
Draw at trough. Every follow-up sample must be at the same point in the weekly cagrilintide cycle, immediately before the next injection, otherwise the drift in serum concentration is confounded with any real physiological change.
Management Strategies
If you are not on exogenous insulin. Standard glycaemic monitoring is enough. Rising HOMA-IR or fasting insulin that stops trending down after week 12 is a signal to look at diet composition and training rather than the drug.
If you are on exogenous insulin. Cut mealtime insulin by 50 percent at initiation, per the pramlintide labelling. Re-titrate over four to eight weeks against CGM data, not against fasting numbers. Any severe hypoglycaemia event within three hours of a meal-time injection is a stop-and-reassess signal.
If you are on a sulfonylurea. Same principle. Reduce sulfonylurea dose empirically and monitor with CGM if available; the sulfonylurea's insulin release plus amylin's glucagon suppression is a hypoglycaemia setup.
If fasting glucose drops below 3.5 mmol/L on two occasions. Only realistic in insulin or sulfonylurea users on cagrilintide. Reduce the mealtime insulin further, or hold the sulfonylurea entirely, before continuing cagrilintide.
Stacking with retatrutide. Retatrutide's glucagon receptor agonism partially compensates for cagrilintide's glucagon suppression, but the compensation is not one-for-one because retatrutide's effect is more on hepatic glucose output than on acute counter-regulatory response to falling glucose. Clinical Significance
Cagrilintide's main insulin effect is indirect and driven by weight loss, which is unremarkable. The safety-critical effect is glucagon suppression, which transfers pramlintide's boxed warning for severe hypoglycaemia to any user combining cagrilintide with exogenous insulin or a sulfonylurea. Non-insulin users can monitor with standard fasting panels; insulin users need a continuous glucose monitor and a mandatory 50 percent mealtime insulin reduction at initiation.
