Orforglipron vs Semaglutide: The No-Fasting Pill vs the Original GLP-1
Orforglipron is a daily small-molecule GLP-1 pill with no food rules that took off 11.2% of body weight at 72 weeks. Semaglutide is the peptide behind Ozempic, Wegovy and the Wegovy Pill, with 14.9% weight loss on the 2.4mg injection and 13.6% on the 25mg tablet. Semaglutide loses more weight and has years of safety data; orforglipron is easier to take but raises heart rate more and causes more GI dropouts.
Overview
Both drugs activate the GLP-1 receptor to cut appetite and slow stomach emptying, but they are built completely differently.
Semaglutide is a modified peptide. It has to be injected weekly (Ozempic, Wegovy), or taken as a tablet with an absorption enhancer on a strictly empty stomach (Rybelsus for type 2 diabetes, the Wegovy Pill 25mg for weight loss). It has been on the market since 2017 and carries the SELECT cardiovascular outcomes data.
Orforglipron (Foundayo, Eli Lilly) is a small chemical molecule that survives digestion. You take one tablet a day at any time, with or without food. The FDA approved it in April 2026 and the MHRA in August 2026. It is not registered in Australia, where the TGA approved the Wegovy Pill on 3 November 2026 instead.
The comparison matters most for people who want a pill: the choice in practice is the Wegovy Pill or orforglipron.

Key Differences
Structure and how you take it:
- Orforglipron: non-peptide small molecule, once daily, any time, no fasting. Half-life about 29 to 49 hours
- Semaglutide: peptide. Weekly injection, or daily tablet on waking with no more than 120mL of water and a 30-minute wait before food, drink or other tablets
Weight loss (no head-to-head obesity trial):
- Orforglipron 17.2mg: 11.2% at 72 weeks (ATTAIN-1), 12.4% if everyone stayed on drug
- Semaglutide 2.4mg injection: 14.9% at 68 weeks (STEP 1)
- Oral semaglutide 25mg: 13.6% at 64 weeks (OASIS 4)
- A Novo-funded indirect comparison put oral semaglutide 25mg about 3 points ahead of orforglipron
Type 2 diabetes (head-to-head, ACHIEVE-3):
- Orforglipron 12mg and 36mg trial doses lowered HbA1c by 1.71 and 1.91 points
- Oral semaglutide 7mg and 14mg (Rybelsus) lowered it by 1.23 and 1.47 points
- Orforglipron won on HbA1c but had more GI events (58 to 59% vs 37 to 45%) and about twice the side-effect dropouts
Heart rate:
- Orforglipron: +4 to 5 bpm on the label; about +7 to 9 bpm in a 2026 network meta-analysis, the highest in the class
- Semaglutide: about +3 to 4 bpm (injection), about +4.5 bpm (oral)
Lean mass share of weight lost:
- Orforglipron: 26.9% (ATTAIN-1 DXA subgroup)
- Semaglutide: about 35% (2026 meta-analysis)
Liver:
- Orforglipron: pooled analysis of 11,220 people found ALT and AST fell on average and no drug-induced liver injury, despite two Pfizer oral small-molecule GLP-1s failing on liver signals
- Semaglutide: ALT tends to fall with weight loss; proven benefit in MASH
Interactions:
- Orforglipron: CYP3A4 substrate. Strong inhibitors (clarithromycin) cap the dose at 9mg; simvastatin capped at 20mg; backup contraception for 30 days after starting and each dose step
- Oral semaglutide: absorption collapses if taken with food or other tablets; raises levothyroxine exposure
When to Use Which
Choose semaglutide if:
- Maximum weight loss and the longest safety record matter most
- You are in Australia and want a registered drug now (injection now, Wegovy Pill once launched)
- You want proven cardiovascular outcome data (SELECT)
- You are stacking stimulants, clenbuterol, T3 or trenbolone and want the smaller heart rate rise
Choose orforglipron if:
- You need a pill and cannot manage a fasting window (shift work, morning thyroxine or oral compounds on waking)
- You are coming off a weekly injection and want an oral maintenance option (ATTAIN-MAINTAIN)
- You live where it is registered (US, UK)
Either way: baseline liver, kidney, lipid, HbA1c and lipase bloods, and a week of morning resting heart rate before starting.
Clinical Context
For most patients semaglutide remains the default: more weight loss, cardiovascular outcome data and a decade of use. Orforglipron's advantage is practical rather than pharmacological. A tablet with no fasting rule removes the adherence problem that limits oral semaglutide, and ATTAIN-MAINTAIN supports it as a step-down from injections. Its costs are more GI dropouts and the largest heart rate rise in the class. Neither drug needs routine liver monitoring on its label, but both carry pancreatitis and gallbladder warnings and both raise lipase without symptoms.
Bodybuilder Context
For lifters the deciding factors are heart rate, muscle and logistics. If you run clenbuterol, T3, trenbolone or heavy stimulants in a cut, semaglutide's smaller heart rate rise is the safer partner. Orforglipron's lean share of weight lost (about 27%) is better than semaglutide's (about 35%) in cross-trial data, but resistance training and 1.6g/kg protein move that number more than the drug choice. The oral semaglutide fasting rule clashes with morning T3, thyroxine and oral compounds; orforglipron does not. Grey-market orforglipron is a small molecule of unknown purity, and the two Pfizer drugs in the same class failed on liver toxicity, so a bootleg version is a poor bet.
