Apitegromab vs ACE-031: why one was approved and the other was abandoned
ACE-031 raised lean mass in healthy volunteers and was still stopped on safety grounds. Apitegromab blocks the same pathway far more narrowly, reached approval, and has never been tested for muscle growth.
Overview
This is the most instructive comparison in the myostatin class, because the two drugs show what happens when you block the pathway precisely versus bluntly.
ACE-031 is a soluble decoy receptor: the activin receptor type IIB fused to an antibody Fc fragment. It sits in circulation and soaks up anything that would bind that receptor. That includes myostatin, but also activin A, GDF-11 and several bone morphogenetic proteins. It worked. In healthy postmenopausal women, a single ascending dose raised lean mass by 3.3 percent and thigh muscle volume by 5.1 percent. Then the Duchenne muscular dystrophy trial was halted over nosebleeds and dilated blood vessels in the skin, plausibly because of that same untargeted binding across the TGF-beta family. Development stopped. It was never approved for anything, and vials still circulate on the grey market.
Apitegromab took the opposite approach. It binds the inactive precursor forms of myostatin and blocks the specific protease step that activates them. It does not touch activin A, GDF-11 or the bone morphogenetic proteins. That narrowness is why it got through where ACE-031 did not.
The catch is that nobody has tested whether the narrow version produces the muscle gain the blunt one did.

Key Differences
Mechanism: ACE-031 is a decoy receptor that intercepts many TGF-beta family ligands. Apitegromab is an antibody that binds only the myostatin precursors.
Off-target reach: ACE-031 blocks activin A, GDF-11 and bone morphogenetic proteins alongside myostatin. Apitegromab does not.
Proven muscle gain in healthy adults: ACE-031 raised lean mass 3.3 percent and thigh muscle volume 5.1 percent in healthy postmenopausal women. Apitegromab has never had this measured, in anyone.
Why development diverged: ACE-031's Duchenne trial was stopped over epistaxis and telangiectasias, adverse events outside muscle. Apitegromab completed Phase 3 and was approved in September 2026.
Effect on blood markers: ACE-031 suppresses FSH and to a lesser degree LH, and the receptor class raises haemoglobin and haematocrit, which matters if you are already running testosterone. Apitegromab's reported changes are a rise in serum latent myostatin, which is just drug-bound myostatin, and creatine kinase rises in 28 percent of treated patients against 12 percent on placebo.
Bone: ACE-031 shifts bone turnover towards formation. Apitegromab carries a fracture warning on its label, 9 percent at the approved dose against 2 percent on placebo, with femoral changes in rat studies. Same pathway, opposite-looking bone signals, which tells you how little is settled here.
Availability: ACE-031 is abandoned but still sold by research suppliers. Apitegromab is approved, but only as an SMA infusion that costs a reported 310,000 US dollars a year.
When to Use Which
Neither belongs in a hypertrophy protocol, for opposite reasons.
ACE-031 has the thing lifters want, documented lean mass gain in healthy adults, and it is still obtainable. It is also the one that was stopped in clinical development over adverse events unrelated to muscle. A drug abandoned by its sponsor after a safety signal, now sold without oversight or purity control, is a bad trade at any dose. Its FSH and LH suppression and its effect on haematocrit also stack badly with testosterone.
Apitegromab is the safer-designed molecule and the one with real regulatory scrutiny, but it is a clinic-administered IV infusion available only on an SMA prescription, and there is no evidence it would add muscle to a healthy adult.
The lesson worth taking is the one the whole class teaches: blocking myostatin reliably increases muscle size, and reliably fails to increase strength in step with it. Bimagrumab showed exactly that in healthy older adults, and apitegromab's own adult trial found preserved lean mass with no matching benefit on strength or sit-to-stand testing.
Clinical Context
The contrast explains the design logic of the whole class. ACE-031 demonstrated that broad blockade of activin receptor signalling produces measurable muscle accretion, and simultaneously demonstrated why that breadth is dangerous: the same receptor family governs vascular and haematological processes, and the Duchenne trial was halted over bleeding-related events rather than anything muscular. Apitegromab was engineered to act only on myostatin precursor activation, which removed those off-target effects and allowed it through to approval. What remains unresolved is whether the narrow mechanism retains the anabolic effect, because apitegromab's trials measured motor function in a disease population and lean mass preservation during weight loss, never hypertrophy in a healthy adult.
Bodybuilder Context
ACE-031 has long been the myostatin compound with genuine appeal on forums, because unlike follistatin it has real human data showing lean mass gain. The problem is that it was abandoned by its sponsor, it suppresses FSH and LH, and its receptor class raises haemoglobin and haematocrit, which compounds the haematocrit problem anyone on testosterone is already managing. Apitegromab's approval has prompted a wave of hope that a clean myostatin blocker has finally arrived, but the drug is an unobtainable hospital infusion and the hypertrophy question it supposedly answers was never asked in any of its trials.
