The proportion of white blood cells that are monocytes, expressed as a percentage of the total. This relative differential value is distinct from the absolute monocyte count and is best interpreted alongside it, since it shifts with changes in other white-cell lines.

PED Notes

A relative value, rarely actionable in isolation. Mild elevation of the monocyte percentage can accompany recovery from infection or chronic low-grade inflammation, which in athletes may reflect ongoing musculoskeletal inflammation or overuse. Not directly affected by AAS. Interpret alongside the absolute count and inflammatory markers such as CRP.

When high

When high (>10%):

  • Usually reflects chronic inflammation, recovery from a recent infection, or chronic tissue repair; in athletes it may mirror overuse injury or ongoing musculoskeletal inflammation
  • Confirm with the absolute monocyte count and correlate with CRP; monocytes alone are rarely a reason to act
  • Supportive: address underlying inflammation with omega-3 (EPA/DHA) 2-3g/day and curcumin 500-1000mg/day with piperine; investigate further if persistent (>3 months) without cause

When low

When low (<2%):

  • A low monocyte percentage is rarely clinically significant on its own and is often a relative change when other white-cell lines are elevated
  • No specific intervention is needed; interpret within the full differential
