The proportion of white blood cells that are basophils, expressed as a percentage of the total. Basophils are the rarest white-cell type, so this value is normally very low. It is the relative differential value, distinct from the absolute basophil count.
PED Notes
The rarest white-cell line and rarely significant in isolation. A mildly raised basophil percentage is almost never meaningful on its own; persistent elevation can occasionally accompany allergic conditions, hypothyroidism, or, rarely, myeloproliferative disorders. Not affected by AAS. Interpret within the full differential.
When high
When high (>2%):
- Mild elevation is almost never meaningful on its own; confirm with the absolute basophil count and interpret within the whole differential
- Persistent elevation may occasionally be seen with allergic disease, hypothyroidism, or, rarely, a myeloproliferative disorder; check TSH if there are corroborating features
- No specific supplement or lifestyle measure targets basophils directly
When low
When low (<0%, effectively absent):
- A low or zero basophil percentage is normal and expected given how rare these cells are; it carries no clinical significance
- No action is required
